| Summary, etc. | Introduction: Locally advanced breast cancer patients represent nearly half of newly diagnosed breast cancer patients in developing countries and despite the different systemic treatment options in neoadjuvant settings they still represent a challenging situation. Applying radiation therapy concurrently with systemic therapy may improve pathological response in such patients and therefore facilitate surgery and improve survival outcomes.<br/>Patient and methods:One hundred sixty locally advanced breast cancer patients were included and randomized into two arms; one receiving neoadjuvant systemic treatment only and the other receiving neoadjuvant concurrent chemoradiation. Surgery was done for both arms as indicated 3-4 weeks post systemic treatment in the control arm and 6-8 weeks post radiation therapy in the interventional arm. Pathologic complete response rates, radiation induced dermatitis and wound complications were observed and compared between both arms.<br/>Results: pathologic complete response was achieved in 46% of the interventional arm compared to 30% of the control arm with a statistically significant difference. Dermatitis was insignificantly different between both arms as well as the wound complications.<br/>Conclusion: the neoadjuvant chemoradiation protocol is feasible with probably promising complete response rates and acceptable toxicity profile. |

| Summary, etc. | تعد حالات اورام الثدي المتقدمه من اصعب الحالات التي تنتشر في الدول الناميه. يكون اختيار خطة العلاج المناسب من حيث الفاعليه و الأمان تحدي كبير.<br/>اثبتت الدراسات ان تلقي العلاج الدوائي قبل الجراحه يحسن من فرص اجراء الجراحات التحفظيه و ان نسب استجابة الورم السرطاني للعلاج تتناسب طرديا مع فرص الشفاء طويل المدي, و ان المرضي الذين يحققوا استجابه كامله بعد تلقي العلاج نسب شفاءهم عاليه. و مع ذلك يظل هناك نسبه لا باس بها لا تستجيب بشكل مرضي للعلاج الدوائي و عليه قد يكون جمع العلاج الكيماوي و الاشعاعي قبل الجراحه حل مثالي لهذه الحالات.<br/>تستهدف هذه الدراسه تلك الحالات من اورام الثدي المتقدمه, و قد قمنا بتقسيم الحالات الي مجموعتين: واحده تتلقي علاج دوائي فقط ثم تجري جراحه يلحق بها العلاج الاشعاعي و الدوائي الوقائي اما المجموعه الأخرى فتلقت علاج دوائي وفقا للارشادات العالميه مصحوبا بالعلاج الاشعاعي للثدي و الغدد الليمفاويه, و تم اجراء الجراحه لكل الحالات بعد انتهاء العلاج. <br/>قد قمنا بمقارنة نتائج العلاج علي المجموعتين كذلك الاثار الجانبيه للعلاج و مضاعفات الجراحه. |
