Summary, etc. Background: Solitary lung cavities are radiological findings characterized by an air-filled space within a pulmonary lesion, often identified through computed tomography (CT). These cavities can arise from a variety of pathological processes, ranging from infectious and inflammatory conditions to primary or metastatic malignancies. Differentiating between malignant and non-malignant etiologies is crucial for timely and accurate diagnosis, which significantly impacts clinical management and patient outcomes.<br/>Aim: This study aimed to evaluate the CT imaging characteristics of solitary lung cavities and correlate these findings with histopathological results to identify distinguishing features between malignant and non-malignant etiologies.<br/>Methods: A cross-sectional study was conducted. The study included 45 patients with solitary lung cavities. It was conducted at Kasr Al Aini hospital. CT scans were reviewed to assess cavity size, wall thickness, margin characteristics, and associated findings such as consolidation, ground-glass opacities, or nodular patterns. The imaging findings were compared with the histopathological data obtained from biopsies, resections, or clinical follow-up. <br/>Results: Initial findings indicated distinct patterns in CT characteristics that may aid differentiation. Malignant cavities typically exhibit irregular margins, a wall thickness of >5 mm, and associated nodular components, whereas non-malignant cavities often demonstrate thin walls (<5 mm), smooth margins, and surrounding inflammatory changes. Histopathological correlation confirmed these patterns.<br/>Conclusion: CT imaging, combined with pathological correlation, provides valuable insights into the etiology of solitary lung cavities. Key imaging features, such as wall thickness, margin irregularity, and associated findings, can guide clinicians toward a probable diagnosis, thus improving patient management. Further studies with larger sample sizes are recommended to validate these findings and enhance the diagnostic algorithms for solitary lung cavities.

Summary, etc. هدفت هذه الدراسة إلى تقييم الخصائص الإشعاعية والمرضية للتجاويف الرئوية الوحيدة، مع التركيز على التمييز بين الآفات الخبيثة والحميدة. شملت الدراسة 45 مريضًا من مستشفى قصر العيني، مع غلبة للذكور (68.9%) ومتوسط عمر 45.8 سنة. أظهرت النتائج أن معظم المرضى لديهم تاريخ سريري لالتهابات صدرية (95.6%) ، بينما كان لدى نسبة أقل (8.9%) تاريخ للإصابة بالأورام الخبيثة. تبين أن العمر كان عاملًا مهمًا في التمييز بين الآفات الحميدة والخبيثة، حيث كان المرضى الأكبر سنًا (متوسط عمر 61.0 سنة) أكثر عرضة للإصابة بالآفات الخبيثة. بينما لم يظهر أن للجنس تأثيرًا كبيرًا على نوع الآفة.<br/>أثبتت صور الأشعة المقطعية أنها أداة تشخيصية دقيقة للغاية، حيث أظهرت اختلافات إشعاعية رئيسية بين الآفات الحميدة والخبيثة. تميزت الآفات الخبيثة بجدران سميكة (>5 ملم)، غير منتظمة، وتجاويف مليئة بالهواء، مع وجود سمات مصاحبة مثل عتامات الزجاج المصنفر وتضخم العقد الليمفاوية. في المقابل، كانت الآفات الحميدة تحتوي على جدران أرق (<5 ملم) ، بمحتوى مختلط من الهواء والسوائل، وعلامات التهابات.<br/>أكد التحليل المرضي أن 82.2% من الآفات كانت حميدة، وكانت السل وأمراض الرئة الالتهابية التشخيصات الأكثر شيوعًا، بينما كانت الآفات الخبيثة (17.8%) غالبًا عبارة عن سرطان غدي وسرطان الخلايا الحرشفية. أظهرت صور الأشعة المقطعية دقة تشخيصية مثالية، حيث حققت حساسية ونوعية واتفاقًا مع التحليل النسيجي بنسبة %100.
