Our study correlated case history, physical exam, hemato-biochemical data, ultrasonography, and <br/>serum cystatin C (Cys-C) to evaluate the diagnostic and prognostic value in liver disease, kidney disease, and <br/>hepatorenal syndrome (HRS) for better management and reduced fatal risk. The studies included 95 dogs: <br/>clinically healthy (n=21), acute kidney disease (n=22), chronic kidney disease (n=7), acute hepatitis (n=17), <br/>chronic hepatitis (n=5), cholangiohepatitis (n=5), cholangiocholecystitis (n=5), liver masses/lymphoma (n=6), <br/>and hepatorenal syndrome (n=7). Samples were collected following thorough clinical and Ultrasonographic <br/>(US) examination at Private Small Animal Clinics and outpatient medical unit of Faculty of Veterinary <br/>Medicine; Cairo University. Hematology revealed anemia, thrombocytosis and leukocytosis in both acute <br/>kidney disease (AKD) and chronic kidney disease (CKD) groups, while serum biochemistry revealed a <br/>significant increase in alkaline phosphatase (ALKP), cholesterol, glucose, ammonia, blood urea nitrogen <br/>(BUN), creatinine (Cr), potassium (K+), phosphorus levels, and Cys-C as well as significant decrease in <br/>BUN/Cr and Na+/K+ ratios in both groups, along with a decrease in calcium value and urine specific gravity <br/>in CKD. Urinalysis revealed a significant increase in urinary protein creatinine (UPC) ratio in both groups. <br/>Hepatobiliary hematological analysis revealed significant decreases in red blood cell parameters, such as RBC <br/>count, hemoglobin (Hb), and mean corpuscular hemoglobin concentration (MCHC), in specific disease groups. <br/>Leukocytosis with neutrophilia and lymphocytosis was observed in several groups, while HRS cases exhibited <br/>neutrophilia and eosinophilia. Biochemical analysis showed significant elevations in liver enzymes (ALT, AST, <br/>ALP, GGT) and bilirubin while decreased cholesterol level in different disease groups. Renal function tests, <br/>including BUN, creatinine, and electrolytes, were significantly altered in HRS cases. US examination identified <br/>specific ultrasonographic markers such as hepatomegaly, hypoechoic liver parenchyma, biliary dilatation, and <br/>renal changes in cases of HRS. Importantly, Cys-C demonstrated higher sensitivity and specificity than <br/>creatinine as a biomarker for renal impairment, particularly in acute and chronic kidney disease and HRS. This <br/>study highlights serum cystatin C as a superior biomarker to creatinine for early detection and prognostication <br/>of renal dysfunction in canine hepatobiliary and renal disorders, particularly hepatorenal syndrome. These <br/>findings support the integration of cystatin C into routine diagnostic panels and provide a foundation for future <br/>research on its role in veterinary nephrology and hepatology. |

أُجريت دراسات لربط السيرة المرضية والفحص الإكلينيكي ونتائج الفحوصات الهيماتو-بيوكيميائية السريرية، مع الشذوذات فوق الصوتية، بالإضافة إلى مستوى السيرم من السيستاتين C كواسم حيوي لتلف الكلى، وذلك لبحث الأهمية التشخيصية والتنبؤية في مختلف أمراض الكبد والكلى أو في الكشف عن متلازمة الكبد الكلوي (HRS)، بهدف تحقيق إدارة علاجية مناسبة وتقليل خطر العواقب المميتة المحتملة. وقد أُجريت الدراسة على 95 كلبًا وُزِّعوا كما يلي: كلاب سليمة إكلينيكيًا (21)، مرض الكلى الحاد (22)، مرض الكلى المزمن (7)، التهاب الكبد الحاد (17)، التهاب الكبد المزمن (5)، التهاب القنوات الصفراوية والكبد (5)، التهاب المرارة والقنوات الصفراوية والكبد (5)، أورام الكبد/ليمفوما (6)، ومتلازمة الكبد الكلوي (7). جُمعت العينات من جميع الكلاب المشاركة بعد فحص سريري شامل للعلامات الحيوية الظاهرة بما يشمل: معدل التنفس والنبض، درجة الحرارة، الأغشية المخاطية، فحص العقد اللمفاوية، والفحص بالموجات فوق الصوتية، وذلك من عيادات خاصة للحيوانات الصغيرة ومن العيادة الخارجية بكلية الطب البيطري – جامعة القاهرة.<br/>أظهرت نتائج أمراض الدم وجود أنيميا، وزيادة في الصفائح الدموية (Thrombocytosis) وزيادة في كريات الدم البيضاء (Leukocytosis) في مجموعتي مرض الكلى الحاد والمزمن، بينما كشفت الكيمياء الحيوية عن زيادة معنوية في إنزيم الفوسفاتاز القلوي (ALKP)، الكوليسترول، الجلوكوز، الأمونيا، نيتروجين يوريا الدم (BUN)، الكرياتينين (Cr)، البوتاسيوم (K+)، والفوسفور، وكذلك السيستاتين C، مع انخفاض معنوي في نسبتي BUN/Cr وNa+/K+ في كلا المجموعتين، بالإضافة إلى انخفاض الكالسيوم والكثافة النوعية للبول في حالات الكلى المزمنة. كما أظهر تحليل البول زيادة معنوية في نسبة البروتين إلى الكرياتينين البولي (UPC) في المجموعتين. علاوة على ذلك، أظهر السيستاتين C حساسية وخصوصية أكبر من الكرياتينين كمؤشر تنبؤي في حالات الكلى الحادة والمزمنة.<br/>أظهر الفحص بالموجات فوق الصوتية للجهاز الكبدي الصفراوي علامات نوعية مثل تضخم الكبد (Hepatomegaly)، انخفاض الصدى في نسيج الكبد (Hypoechoic parenchyma)، توسع القنوات الصفراوية (Biliary dilatation)، والتغيرات الكلوية في حالات متلازمة الكبد الكلوي. وأظهرت التحاليل الدموية انخفاضًا معنويًا في مؤشرات كريات الدم الحمراء مثل عدد كريات الدم الحمراء (RBCs)، تركيز الهيموجلوبين (Hb)، وتركيز الهيموجلوبين الوسطي في الكرية (MCHC) في بعض مجموعات الأمراض. كما لوحظت زيادة في كريات الدم البيضاء مع عدلات (Neutrophilia) ولمفاويات (Lymphocytosis) في عدة مجموعات، بينما أظهرت حالات متلازمة الكبد الكلوي زيادة في العدلات والحمضات (Eosinophilia). وأظهر التحليل الكيميائي الحيوي ارتفاعًا معنويًا في إنزيمات الكبد (ALT, AST, ALP, GGT) والبيليروبين مع انخفاض الكوليسترول في بعض مجموعات الأمراض. وقد تأثرت اختبارات وظائف الكلى بشكل ملحوظ في حالات متلازمة الكبد الكلوي، بما في ذلك BUN والكرياتينين والشوارد. ومن الجدير بالذكر أن الواسم الالتهابي الكلوي؛ السيستاتين C ارتفع بشكل كبير في مجموعة الـ HRS، مما يبرز قيمته التنبؤية المحتملة في تقييم تلف الكلى. |
