Summary, etc. Background: <br/>Kidney stones represent a significant health issue in children, with a globally increasing <br/>incidence. The rising incidence is attributed to various factors such as increased sensitivity of <br/>radiological imaging, systematic screening for metabolic risk factors, changes in nutrition <br/>patterns, consanguinity and rise in childhood obesity. <br/>Aim of the Study: <br/>This study aims to study renal stone disease in Egyptian children attending the Pediatric <br/>Nephrology Outpatient Clinic of Cairo University Children's Hospital (CUCH). It also seeks <br/>to document the epidemiological characteristics of the disease in Egyptian children and to <br/>determine the possible causes, management, and short-term outcomes. <br/>Methods: <br/>A retrospective descriptive study of 128 patients (<13 years) with kidney stones <br/>attending Cairo University Children’s Hospital (2015–2019). Data were collected from medical <br/>records, focusing on demographics, clinical presentation, imaging findings, management and <br/>outcome (CKD staging). Laboratory investigations included complete blood count, serum <br/>electrolytes (Na, K, Ca, Po4, ALP, Mg), blood gases, BUN and serum creatinine, serum uric <br/>acid,and parathormone ,stone, and urine analysis. Data were analyzed using SPSS version 26, <br/>employing Chi-Square, Fisher's exact test, Mann Whitney U test, and Spearman's correlation <br/>coefficient. <br/>Results: <br/> The median age of the children ranged from 0.3 to 11.0 years, with a median of 4 <br/>years. The majority were males (74.2%), and 63.3% were from rural areas. Results showed that <br/>60.2% of patients had consanguineous parents, and 50% had a family history of a similar <br/>condition. Renal colic was the most common symptom (25.8%), followed by anuria (14.8%), <br/>and recurrent urinary tract infection (10.9%). The median age of symptom onset was 2 years <br/>(0.1-11 years). Radiological studies revealed that 42 patients (32.8%) had nephrocalcinosis. <br/>Calcium oxalate stones were the most common type (33.6%). The recurrence rate was 19.5%. <br/>Age at diagnosis and age at symptom onset were significantly lower in nephrocalcinosis cases <br/>compared to kidney stone cases. Hypercalciuria was significantly more common in patients <br/>with stone recurrence. Passage of gravels or small stones (P value=0.001) was significantly <br/>higher in patients with uric acid stones. All patient with cystine stone were sibling of <br/>consanguineous marriage. GFR was significantly lower in obstructive group than non-<br/>obstructive (P value=0.001). There was a negative correlation between Ca/ creatinine ratio and <br/>GFR (r = -0.426, P value =0.001). <br/>Conclusion: <br/> Calcium oxalate dominates pediatric stones in Egypt, nephrocalcinosis is a prevalent <br/>condition with consanguinity and rural residence as key risk factors. Obstructive stones and <br/>recurrence correlate with renal impairment. Tailored prevention strategies and multicenter <br/>studies are recommended.

Summary, etc. المقدمة:<br/>مرض حصوات الكلى هو اضطراب أيضي جهازي يرتبط بزيادة مخاطر الإصابة بأمراض القلب والأوعية الدموية، وأمراض الكلى المزمنة لدى البالغين، وكذلك بزيادة خطر حدوث الكسور لدى الأطفال والبالغين.<br/>تتكوَّن حصوات الكلى نتيجة زيادة تركيز المواد الذائبة المعنية، وتُسهم عوامل متعددة في تجمّع هذه المواد والتصاقها بخلايا الأنابيب الكلوية، مثل درجة حموضة البول، عدم كفاءة المثبطات الطبيعية للتبلور، ركود البول، بالإضافة إلى تلف الأنابيب الكلوية .(<br/>في الآونة الأخيرة، لوحظ ارتفاع تدريجي في معدل حدوث حصوات الجهاز البولي لدى الأطفال، خاصة في الدول المتقدمة، حيث أُبلغ عن زيادة في حالات حصى الكلى بمقدار خمسة أضعاف في كل من أمريكا الشمالية وتركيا.<br/>تُعد منطقة الحزام الحجري الأفروآسيوي، من المناطق ذات المعدلات العالية من الإصابة والانتشارلمرض حصوات الكلى، وتمتد من السودان ومصر في إفريقيا إلى المملكة العربية السعودية، وإيران، وتركيا، وباكستان، والهند في آسيا.<br/>يمكن تفسير هذا الارتفاع في معدل الإصابة بعدة عوامل، منها: زيادة دقة التصوير الشعاعي، الفحص المنهجي لعوامل الخطر الأيضية، التغيرات في النمط الغذائي، زواج الأقارب، وارتفاع معدلات السمنة لدى الأطفال. كما يُعتقد أن الاحتباس الحراري وتغير المناخ يسهمان أيضًا في ذلك. معظم الحالات تكون مرتبطة باضطرابات أيضية .<br/>تعتبر حصوات أوكسالات الكالسيوم هي الأكثر شيوعًا لدى الأطفال، حيث تشكل نسبة 70–80٪ من جميع حصوات الجهاز البولي، بينما تُعد حصوات حمض اليوريك، الستروفيت، والسيستين أقل شيوعا.<br/>الهدف من الدراسة:<br/>هدفت هذه الدراسة إليدراسة مرض حصوات الكلي في الأطفال المترددين علي العيادات الخارجيه لوحده الكلي بمستشفيات جامعة القاهرة ولتوثيق الخصائص الوبائية للمرض لدى الأطفال المصريين وتحديد الأسباب المحتملة والعلاج والنتائج قصيرة المدى.
