| Summary, etc. | Objective: The study conducted a comparative analysis of the <br/>effectiveness and safety of intrauterine misoprostol compared to rectal <br/>misoprostol plus oxytocin to decrease blood loss during and following <br/>cesarean section (CS) procedures. <br/>Methodology: In this randomized controlled study, 200 pregnant women <br/>candidates for lower segment CS were randomly assigned to two equal <br/>groups; the intrauterine misoprostol received 400 mcg of misoprostol with <br/>200 mcg inserted in each uterine cornu intraoperatively following delivery <br/>of the placenta, the rectal misoprostol group received 400 mcg of <br/>misoprostol rectally preoperatively following insertion of the urinary <br/>catheter, the two groups received oxytocin infusion of 20 IU in a 500 ml <br/>Lactated Ringer solution at a rate of 125 ml/hr . The primary outcome was <br/>the estimated blood loss (EBL) after cesarean birth. The secondary outcome <br/>measures were the need for ecbolics denoting uterine atony, excessive blood <br/>loss (> 1000 mL), need for blood transfusion, and maternal side effects. <br/>Results: There is a no significant difference in EBL in both the intrauterine <br/>and rectal misoprostol group. No significant intergroup differences were <br/>observed regarding secondary outcomes <br/>Conclusion: Intrauterine and rectal Misoprostol are equally effective <br/>regarding bleeding during and after CS, although not to an extent to <br/>clinically decrease maternal morbidity. |

| Summary, etc. | نزيف ما بعد الولادة من المشاكل المؤرقة حول العالم و تؤدي الي عواقب صحية كارثية تصل الي وفاة الأم. ومن المثبت أن الولادة القيصرية تجعل الأم أكثر عرضة للنزف مقارنة بالولادة الطبيعية.<br/>يعتقد أن بعض العقارات المقوية لانقباض الرحم مثل الاكسيتوسين و الميزوبروستول يساعدون في تقليل الدم أثناء و بعد القيصرية.<br/>الهدف من هذه الدراسة: هو بيان و فعالية الميزوبروستول الرحمي مقارنة بالشرجي بالاضافة للاكسيتوسين في الحد من فقد الدم أثناء و بعد الولادة القيصرية.<br/>تم هذا البحث علي 200 سيدة قسمن بالتساوي علي ثلاث مجموعات:<br/>المجموعة الاولي: أخدت الميزوبروستول الرحمي بالاضافة الي الاكسيتوسين<br/>المجموعة الثانية: أخدت الميزوبروستول الشرجي بالاضافة الي الاكسيتوسين<br/>البحث تم اجراءه علي السيدات عشوائيا و تم على سيدات في سن ما بين 20 -40 سنة و تقرر لهم إنهاء الحمل عن طريق الولادة القيصرية تحت تأثير المخدر النصفي.<br/>تم أخذ موافقة كتابية من المرضي بعد عمل فحص شامل و بعد عمل التحاليل الطبية المطلوبة و في هذه الدراسة تمت متابعة أولية لكمية الدم المفقودة في المجموعتين و متابعة ثانوية في الاحتياج إلي أدوية أخري قابضة للرحم و حدوث نزيف مهبلي شديد و الاحتياج لنقل الدم خلال 24 ساعة من العملية مع متابعة أي مضاعفات للأم.<br/>أهم النتائج التطبيقية التي توصل اليها البحث هي أن الميزوبرستول سواء الشرجي أو الرحمي بالاضافة الي الاكسيتوسين يؤدي إلي نفس فقد دم بينما جميع المقارنات الاخري تعتبر متساوية احصائيا و المتابعة الثانوية أيضا أوضحت أنه لا فرق بين المجموعات في شدة النزف أو الاحتياج لعلاجات إضافية أو نقل الدم. |
