| Summary, etc. | Purpose: <br/>To evaluate early microvascular changes in the retina and optic nerve head using optical <br/>coherence tomography angiography (OCTA) in patients undergoing ruthenium-106 (Ru-106) <br/>episcleral plaque brachytherapy for intraocular tumors, and correlate these changes with <br/>radiation dose, tumor location, and visual acuity outcomes. <br/>Methods: <br/>This interventional case series included 30 eyes of 30 patients with extramacular intraocular <br/>tumors treated with Ru-106 plaque brachytherapy at Kasr Al-Ainy Hospital between March 2023 <br/>and March 2025. OCTA was used to assess changes in peripapillary nerve fiber layer plexus <br/>capillary density (NFLP-CD), macular superficial and deep capillary plexus vessel density (SCP-<br/>VD and DCP-VD), and foveal avascular zone (FAZ) at baseline, 3 months, and 6 months. <br/>Radiation dose to the macula and optic nerve head was calculated using Plaque Simulator™, and <br/>its correlation with vascular parameters and visual acuity was analyzed. <br/>Results: <br/>A significant decline in NFLP-CD was observed at 3 and 6 months (p < 0.001). Macular SCP-<br/>VD showed an initial drop at 3 months followed by partial recovery at 6 months, while macular <br/>DCP-VD declined progressively with statistically significant reduction at 6 months. FAZ area <br/>showed significant enlargement over time. Visual acuity deterioration correlated with reduced <br/>vascular density and higher radiation dose to optic nerve. Pre-equatorial tumors were associated <br/>with greater SCP-VD loss. <br/>Conclusion: <br/>OCTA detects early microvascular alterations following Ru-106 brachytherapy. SCP shows <br/>transient drop, while DCP damage is progressive. These changes are dose-dependent and <br/>correlate with tumor location and visual outcomes. OCTA serves as a valuable tool for early <br/>detection and follow-up in post-brachytherapy patients. |

| Summary, etc. | هدفت هذه الدراسة إلى تقييم التغيرات الميكروفسيولوجية المبكرة في الأوعية الدموية باستخدام التصوير الضوئي التوافقي ، عقب العلاج الإشعاعي الكثبي للأورام داخل العين بالنظير المشع الروثينيوم -106 <br/>تم إدراج ثلاثين مريضًا يعانون من أورام خارج البقعة ومتابعتهم لمدة ستة أشهر. أظهر التصوير الضوئي انخفاضاً ملحوظًا في كثافة الشعيرات الدموية حول العصب البصري وفي مقولة العين، خاصة في الضفيرة الشعرية العميقة. وقد وُجدت علاقة قوية بين جرعات الإشعاع التي تصل إلى المقولة والعصب البصري وبين تدهور حدة الإبصار وحدوث تلف في الأوعية الدموية<br/>أظهرت الأورام الواقعة في الجزء الخلفي من العين تراجعًا أكبر في كثافة الضفيرة حول العصب البصري ، بينما ارتبطت الأورام الأمامية بتلف أكبر في الضفيرة السطحية.<br/>كما وُجد تفاوت في شدة الإصابة الإقفارية بين أجزاء الشبكية المختلفة وفقًا لقربها من موضع القرص الإشعاعي، بالإضافة إلى اختلافات في كثافة الأوعية الدموية بين هذه الأجزاء قبل بدء العلاج، حيث لوحظت مناطق إقفارية في المناطق القريبة من الورم.<br/>وقد أثبتت تقنية التصوير الضوئي التوافقي فعاليتها كأداة حساسة وغير باضعة للكشف المبكر عن اعتلال الأوعية الدموية الناتج عن الإشعاع والمضاعفات التي قد تهدد البصر بعد العلاج الإشعاعي الموضعي أو الكثبي. |
