| Summary, etc. | Background: Kidney transplantation (KT) is considered as the best therapeutic <br/>option for patients with end stage kidney disease (ESKD), Tacrolimus (Tac) is a <br/>mainstay in the maintenance immunosuppression and plays a main role in preventing <br/>allograft rejection, however, it is associated with a narrow therapeutic range, <br/>maintaining Tac concentration in blood within the therapeutic range is essential to <br/>avoid over or under immunosuppression, Tac intrapatient variability (IPV) is a new <br/>emerging marker that may affect the graft outcome. <br/>Aim of the work: The study aims to investigate if there is an association between <br/>early Tac IPV and graft survival in KT recipients with low immunological risk, also <br/>to determine whether high IPV is associated with inferior graft outcome or not. <br/>Patients and Methods: A prospective analytical study was performed on ninety-<br/>five KT recipients with low immunological risk, Tac trough levels were recorded <br/>from month one till the end of year one, Tac IPV was assessed using coefficient of <br/>variation (CV), Graft function one year post-transplant was estimated using <br/>estimated glomerular filtration rate (e-GFR), Doppler ultrasonography and strain <br/>elastography were performed at the end of the first year and statistical analysis was <br/>done. <br/>Results: The analysis showed that Tac IPV calculated using CV is significantly <br/>correlated with the graft function one year post transplant, (r = -0.219; p-value = <br/>0.033), Tac CV is positively correlated with strain elastography at year one (r = <br/>0.129; p-value = 0.213) and resisitivity index (r = 0.168; p-value = 0.104). <br/>Conclusion: KT recipients with high Tac IPV are at higher risk for decline in graft <br/>function at one year post transplant, |

| Summary, etc. | يعتبر دواء التاكروليموس أحد أهم الأدوية المثبطة للمناعة المستخدمة فى حالات زرع الكلى ومع ذلك، فإن له نطاق علاجي ضيق ولذلكتعد مراقبة مستوى التاكروليموس فى الدم أمرًا ضروريًا للحفاظ على مستوى الواء فى الدم في النطاق العلاجي،ويعتبر تغير مستوى التاكروليموس داخل المريض بمثابة علامة جديدة قد يكون لها دور محتمل في التنبؤ بتدهور وظائف الكلى المزروعة و يمكن استخامها فى تحديد المرضى الأكثر عرضة لارتفاع وظائف الكلى أو حدوث رفض للكلى المزروعة.<br/>شملت الدراسة خمسة وتسعين مريض من مرضى زرع الكلى ذوي المخاطر المناعية المنخفضةوتمت متابعتهم من الشهر الأول بعد الزراعة حتى نهاية السنة الأولى. تم تسجيل جميع مستويات التاكروليموس فى الدم أثناء المتابعة وتم حساب تغير مستوى التاكروليموس داخل المريض عن طريق معامل التباين و مقارنته بوظائف الكلى بع مرور عام من عملية الزرع و تم إجراء التحليل الإحصائى. <br/>أظهرت النتائج أن تغير مستوى التاكروليموس داخل المريض كان مرتبطًا بعلاقة عكسية كبيرة مع وظيفة الكلى بعد عام واحد من عملية الزرع.<br/>يمكن استخدام تغير مستوى التاكروليموس داخل المريض لتحديد المرضى الأكثر عرضة لتدهور وظائف الكلى ويفضل حسابه لمرضى زرع الكلى للسماح بمتابعة أفضل و تحديد الجرعات اللازمة للحفاظ على الكلى المزروعة. |
