| Summary, etc. | Introduction:Pancreaticoduodenectomy (PD) or Whipple’s procedure remains the standard of treatment for selected patients with periampullary and pancreatic head tumors. During the last few years, the technique has significantly improved, and it is now considered a safe procedure with a hospital mortality rate less than 5% in high-volume centers. The technique is usually accompanied by a relatively high incidence of postoperative complications due to the wide range of surgical resection and different techniques of alimentary tract reconstruction establishment ranging from 45% to 60 %.<br/>Patient and methods:This is a Randomized Controlled Study including 20 patients who underwent pancreaticoduodenectomy with either simple loop or Roux-en-Y reconstruction for periampullary and pancreatic head tumors in the Department of Surgical Oncology of the National Cancer Institute of Egypt (NCI-E) from June 2022 to December 2023. We were comparing between the two methods of reconstruction, either simple loop or Roux-en-Y reconstruction, in preventing postoperative complications and Mortality.<br/><br/>Results: The study included 20 patients operated with pancreaticoduodenectomy (PD) over a span of 8 months by our surgical oncology department of the National Cancer Institute of Egypt (NCI-E) and randomized to simple loop (n = 10) or roux en-Y (n = 10) reconstruction group. <br/>Pancreatic fistula was observed in 35% of our patients Bile reflux wasnoted in 40% of our study cases which complicated with barrets esophagitis in 10% patients in simple loop group by endoscopy and we found that this complication was 10% in Roux en Y group. the RY group doing significantly better than the SL group as assessed objectively by gastric emptying scintigraphy. the simple loop group has a postoperative hypoalbuminemia in comparison to the other R-Y group.<br/><br/>Conclusion:The Roux-en-Y reconstruction, maximizing the use of the proximal jejunum which is rich in motilin receptors, helps in decreasing the incidence and severity of delayed gastric emptying following pancreaticoduodenectomy. The lesser duration of hospital stay and early initiation of adjuvant therapy is an additional benefit with the Roux loop reconstruction. Therefor our study in line with the literature as regarding the effect of reconstruction methods either simple loop or reux-en-y on delayed gastric emptying with the superiority of the later method in preventing this common complication DGE. |

| Summary, etc. | تظل عملية استئصال البنكرياس والاثني عشر أوعملية ويبل هي المعيار العلاجي لمرضى مختارين مصابين بأورام الأثني عشر ورأس البنكرياس. خلال السنوات القليلة الماضية، تحسنت التقنية بشكل كبير، وهي تعتبر الآن إجراءً آمنًا مع معدل أقل من 5٪ في المراكز ذات المعدل الكبير لإجراء تلك الجراجة المعقدة. عادة ما تكون التقنية مصحوبة بنسبة عالية نسبيًا من المضاعفات بعد الجراحة بسبب النطاق الواسع للاستئصال الجراحي والتقنيات المختلفة لإعادة بناء الجهاز الهضمي تتراوح من 45٪ إلى 60٪.<br/>هذه الدراسة العشوائية المحكومة تضم 20 مريضًا خضعوا لاستئصال البنكرياس والاثني عشر إما بإعادة بناء الحلقة البسيطة أو إعادة بناء المجازة المَعِدية لأورام الأثني عشر ورأس البنكرياس في قسم جراحة الأورام بالمعهد القومي للأورام في مصر خلال الفترة من يونيو 2022 إلى ديسمبر 2023 للمقارنة بين طريقتي إعادة البناء، في منع المضاعفات بعد الجراحة والوفيات.<br/>لقد أظهرت الدراسة النتائجإن إعادة بناء المجازة المَعِدية والتي تعظم استخدام الجزء القريب من الصائم الغني بمستقبلات الموتيلين، تساعد في تقليل حدوث وشدة تأخر إفراغ المعدة بعد استئصال البنكرياس والاثني عشر. كما أن تقصير مدة الإقامة في المستشفى والبدء المبكر في العلاج المساعد من الفوائد الإضافية لإعادة بناء المجازة المَعِدية.<br/>إن إعادة بناء المجازة المَعِدية مع تأخير إفراغ المعدة كانت أفضل بكثير من مجموعة إعادة بناء الحلقة البسيطة كما تم تقييمها بشكل موضوعي من خلال التصوير المقطعي لإفراغ المعدة.<br/>ومع ذلك، هناك حاجة إلى دراسة مستقبلية أكبر لمعالجة هذا السؤال. لذلك فإن دراستنا تتماشى مع الدراسات السابقة فيما يتعلق بتأثير طرق إعادة البناء سواء الحلقة البسيطة أو إعادة بناء المجازة المَعِدية على تأخر إفراغ المعدة مع تفوق الطريقة اللاحقة في منع هذه المضاعفات الشائعة وهي تأخير إفراغ المعدة. |
