| Summary, etc. | In this study it was found that both set of patients achieved consistently better scores throughout the study period. The surgically treated patients achieved higher scores, although both groups had a close time to full weight bearing on average, the operative group had an earlier return to work. These results are consistent with several studies across the literature.<br/>Since the articular surface of the posterior facet of the calcaneus in these comminuted fractures is non reconstructable in these cases, we recommend primary subtalar arthrodesis, as it provide a better a means of returning to work faster, achieve better functional outcome in a shorter |

| Summary, etc. | تشكل كسور عظمة العقب المفتتة تحدياﹰ للمريض و للجراح المعالج، إذ يعد العجز طويل الأمد و عدم القدرة على العودة للعمل و الأنشطة اليومية ما قبل الأصابة من المضاعفات الناتجة عن هذه الكسور و معالجتها.<br/>سؤال البحث: هل يحقق التثبيت الداخلي بالرد المفتوح مع الإندماج المفصلي الأولي للمفصل الكنزعي نتائج وظيفية أفضل مقارنة بالعلاج التحفظي في كسور عظمة العقب المفتتة؟<br/>المنهجية: تم تصميم دراسة تجربة سريرية عشوائية محكومة، لمقارنة النتائج الوظيفية بين العلاج الغير جراحي التحفظي و التثبيت الداخلي بالرد المفتوح مع الأندماج المفصلي الأولي للمفصل الكنزعي. تم مقارنة الناتج الوظيفي بإستخدام AOFAS و FAOS و القدرة على العودة للعمل في خلال سنتين.<br/>النتائج: تشير النتائج إلى وجود دلالة إحصائية في درجات AOFAS و FAOS بين المجموعتين في صالح العلاج الجراحي, على مدار فترة الدراسة. حيث ان العلاج الجراحي يتيح للمرضى تحسن وظيفي و عودة للعمل اسرع من العلاج التحفظي.<br/>القيود: تشمل قيود الدراسة حجم العينة الصغير و غياب بيانات المتابعة طويلة المدى إلى مابعد عامين, مما يحد من القدرة على تقييم النتائج المستدامة.<br/>الخلاصة: يحقق التثبيت الداخلي بالرد المفتوح و الإندماج المفصلي الاولي للمفصل الكنزعي عودة اسرع للعمل و الانشطةاليومية مع نتائج وظيفية افضل بالمقارنة بالعلاج الغير جراحي لكسور عظمة العقب المفتتة. |
