| Summary, etc. | Acute unstable ACJ dislocations (Rockwood III-VI) are a significant surgical <br/>challenge. This meta-analysis compared rigid hook plate (HP) fixation to flexible <br/>suspensory fixation (SF). SF demonstrated superior functional outcomes, less pain, <br/>higher patient satisfaction, and shorter hospital stays. While HP provided better <br/>initial anatomical reduction and lower re-dislocation rates, it had a significantly <br/>higher complication burden, including near-mandatory need for a second surgery <br/>to remove hardware. Consequently, SF emerges as the preferred first-line surgical <br/>option for most patients, prioritizing functional recovery and avoiding re-operation. <br/>HP should be reserved for selective cases where maximal initial stability is <br/>paramount, with full patient awareness of its risks. |
