| Summary, etc. | Background: <br/>The high incidence of PONV after bariatric surgery is referred to several factors such as obesity, prevalence of hiatal hernia, direct gastric irritation by surgical trauma, blood, and secretions, abdominal insufflation, increased surgical duration of > 1 hour, and the use of opioids.<br/>The latest consensus guidelines for prophylaxis against PONV recommended the use of > 2 prophylactic drugs in patients with > 2 risk factors for PONV.<br/>The first and second lines for prophylaxis are commonly 5HT-3 antagonist (e.g., ondansetron) and dexamethasone. However, there is no clear evidence for the superiority of either of the 3rd line drugs over the other. several reports used GRV as an index of the prokinetic efficiency of antiemetic drugs.<br/>Methods:<br/> In this study, participants were randomized into 2 groups:<br/>Group (A) Cyclizine group (n=38): received intravenous Cyclizine (50 mg) in a 10 mL syringe. <br/>Group (B) Metoclopramide group (n=38): participants received intravenous Metoclopramide (10 mg) in a (10 mL) syringe.<br/>in both groups, all patients received intravenous dexamethasone (8 mg) and ondansetron (8 mg) according to local protocols.<br/>A written informed consent was obtained from all participants before inclusion. Randomization was performed using computer generated sequence and concealed by serially numbered, opaque, and sealed envelopes. <br/>Each participant from different groups was obtain a total equal volume of drugs to prevent bias.<br/>On operation day, the following data were recorded: participants demographics (age, gender, weight, height, BMI), medical history, duration of fasting. Serial ultrasonographic of gastric antrum of the patients were done and recorded preoperative before each drug dose was administered and one hour after each drug administration.<br/>The quantitative assessment of the gastric volume was performed by measuring the cross-sectional area (CSA) of the gastric antrum (antral CSA) between peristaltic contractions, using the free-tracing caliber of the ultrasound unit.<br/>Results: <br/>Seventy-six patients were screened for eligibility, (38 patients in each group). <br/>Preoperative Estimated Gastric volume(ml) after drug administration by measure D1 & D2 of gastric antrum at baseline & 1 hour after drugs administration. Both groups showed no statistical significance.<br/>Positive attacks of PONV didn’t show statistically significant differences between the studied groups.<br/>Similarly, there wasn’t statistically significant differences between the studied groups regarding the number of patients who received rescue antiemetic boluses & drug’s complications.<br/> Conclusion: <br/>In morbidly obese patients, the use of both Metoclopramide or Cyclizine in combination therapy for prevention of PONV and decrease GRV is similarly effective |

| Summary, etc. | يقوم المرضى الذين يخضعون لعملية جراحية لعلاج السمنة بالإبلاغ عن وجود غثيان وقيء شديدين بعد العملية (PONV)، مما يساهم في تأخير تناول الطعام والحركة بعد العملية، وبالتالي يطيل مدة البقاء في المستشفى. يُشار إلى ارتفاع معدل حدوث الغثيان والقيء بعد عملية السمنة إلى عدة عوامل، مثل السمنة، انتشار فتق الحجاب الحاجز، التهيج المباشر للمعدة بسبب الصدمة الجراحية، الدم، والإفرازات، النفخ في البطن، زيادة مدة العملية لأكثر من ساعة، واستخدام الأفيونات. <br/>وأوصت أحدث الإرشادات التوافقية للوقاية من الغثيان والقيء باستخدام أكثر من دواء وقائي واحد في المرضى الذين لديهم أكثر من عامل خطر واحد لـ PONV .<br/>لقد تم قياس حجم المحتوى المعدي المتبقي (GRV) على نطاق واسع في الطب perioperative خلال السنوات الأخيرة، خاصة مع الاهتمام المتزايد بالموجات فوق الصوتية الموجهة في نقاط الرؤية (point-of-care ultrasound). يُعد GRV مؤشرًا على احتمالية حدوث PONV. علاوةً على ذلك، أبلغت العديد من التقارير عن استخدام GRV كمؤشر على كفاءة الأدوية الممكّنة للحركية المعوية .<br/>هدف هذه الدراسة هو تقييم تأثيرات الدواءين: ميتوكلوبراميد وسيكلزين، ضمن سياق استراتيجية متعددة الأدوية لمكافحة الغثيان والقيء، على GRV لدى المرضى الم scheduled لإجراء جراحة السمنة. تم فحص 76 مريضًا للتأهل، حيث تم تخصيص 38 مريضًا لكل مجموعة.<br/>وجدنا أن استخدام ميتوكلوبراميد أو سيكلزين معًا في علاج وقائي لمرضى السمنة للحد من PONV وتقليل GRV هو فعال بشكل متساوٍ. |
