| Summary, etc. | Background: Breast cancer (BC) is the most frequently diagnosed cancer and is the leading <br/>cause of cancer deaths in females worldwide, Preoperative neoadjuvant chemotherapy has <br/>become a commonly used clinical treatment for BC in recent years; Circulating microRNAs <br/>(miRNAs) have been indicated as predictive biomarker for the response to neoadjuvant <br/>chemotherapy (NAC) and the prognosis of breast cancer (BC). <br/>The aim of the present work: is to evaluate the potential of micro- RNA 34a as a non-<br/>invasive biomarker for detecting neoadjuvant chemotherapy response in Egyptian female with <br/>breast cancer. <br/>Subjects and methods: this study was conducted on 40 newly diagnosed proven breast <br/>cancer patients; determination of serum expression level of MiR34a was done before the start of <br/>neoadjuvant chemotherapy and after the second cycle of the therapy after normalization to MiR-<br/>16using the delta CT method. <br/>Results: The results of the present study showed a statistical significant down-<br/>regulation of the relative expression of MiR34a after the second cycle of <br/>neoadjuvant chemotherapy from baseline relative expression in responders with <br/>luminal b subtype with p value = 0.046. Also, responders with positive Estrogen <br/>Receptors (ER) status showed a statistical significant down-regulation of the <br/>relative expression of MiR34a after the second cycle of neoadjuvant chemotherapy <br/>from baseline relative expression with p value =0.05 compared to its base line <br/>relative expression (C0).However there was no significant change in the relative <br/>expression of MiR 34a after the second cycle of neoadjuvant chemotherapy from <br/>base line relative expression in the whole patient group responders versus non <br/>responders . <br/>Conclusion: Dynamic changes in MiR34a detected in ER positive responders and <br/>luminal B complete responders after the second cycle of NAC administration in <br/>breast cancer patients compared to its baseline expression may reflect its <br/>usefulness as a predictive biomarker to NAC response |

| Summary, etc. | سرطان الثدي (BC) هو السرطان الأكثر تشخيصًا وهو السبب الرئيسي لوفيات السرطان بين الإناث في جميع أنحاء العالم، وقد أصبح العلاج الكيميائي المساعد الجديد قبل الجراحة علاجًا سريريًا شائع الاستخدام لمرض BC في السنوات الأخيرة؛ تمت الإشارة إلى الرناوات الدقيقة المنتشرة (miRNAs) كمؤشر حيوي تنبؤي للاستجابة للعلاج الكيميائي المساعد الجديد (NAC) والتشخيص لسرطان الثدي (BC).<br/><br/>الهدف من العمل الحالي: هو تقييم إمكانات micro-RNA 34a كمؤشر حيوي غير جراحي للتنبؤ باستجابة العلاج الكيميائي المساعد الجديد لدى الإناث المصريات المصابات بسرطان الثدي.<br/><br/>المواضيع والطرق: أجريت هذه الدراسة على 40 مريضة تم تشخيص إصابتهن بسرطان الثدي حديثاً؛ تم تحديد مستوى التعبير النسبي في الدم لـ MiR34a بواسطة qRT-PCR قبل بدء العلاج الكيميائي المساعد الجديد وبعد الدورة الثانية من العلاج بعد التطبيع إلى MiR-16 باستخدام طريقة Delta CT<br/><br/>لنتائج: أظهرت نتائج الدراسة الحالية عدم وجود تغيير كبير في التعبير النسبي لـ MiR34a بعد العلاج الكيميائي المساعد الجديد من التعبير النسبي الأساسي لدى المستجيبين وغير المستجيبين ولا المستجيبين المرضيين الكاملين والمستجيبين المرضيين غير الكاملين. ومع ذلك، كان هناك انخفاض كبير في التعبير النسبي MiR 34a بعد الدورة الثانية من العلاج الكيميائي المساعد الجديد مقارنة بالتعبير النسبي لخط الأساس في المستجيبين الذين لديهم حالة ER إيجابية مع (الوسيط = 0.29، IQR = 0.08 - 1.1 وقيمة p = 0.05) أيضًا كما هو الحال في المستجيبين المرضيين الكاملين من النوع الفرعي اللمعي B مع (الوسيط = 0.41، معدل الذكاء = 0.12 - (1.73 وقيمة P = 0.046)، في حين لم يتم تحديد هذه التغييرات في غير المستجيبين والمستجيبين المرضية غير كاملة على التوالي.<br/>الاستنتاج: التغيرات الديناميكية في MiR34a المكتشفة في المستجيبين الإيجابيين لـ ER والمستجيبين المرضيين اللمعيين B بعد الدورة الثانية من إدارة NAC في مرضى سرطان الثدي مقارنة بتعبيرها الأساسي قد تعكس فائدتها كمؤشر حيوي تنبؤي لاستجابة NAC |
