| Summary, etc. | Objectives<br/> Arthroscopic inferior capsular shift for Multidirecional instability has a good safety profile. Open surgery has been used to decrease recurrence rates with a higher complication rate. Arthroscopic capsular shift was designed to decrease the recurrence rates without the added morbidity of open surgery.<br/>Methods:<br/>Twenty one patients antero-inferior shoulder instability was under went capsular shift by gelnoid anchor after doing the necessary preoperative clinical and radiological assessment according to the following techniques. Through the duration From (April) 2021,to (July) 2024 to a prospective case series study was done for arthroscopic inferior capsular shift by glenoid anchor in antero-inferior shoulder instability.The follow up period of the cases year, twenty one patients met the following inclusion.<br/>Results:<br/> There was statistically significant improvement in The patients constant shoulder score which before surgery ranged from 52 – 68 with (mean60.97 ± 6.01), compared to after year ranged from 82 – 98 with (mean 90.10 ± 5.38), this indicates a clear improvement of cases through constant shoulder score.<br/>The patients American Shoulder and Elbow Surgeons Score improved from 31 – 55 with (mean 45.30 ± 9.56) before surgery to after 1 year ranged from 87 – 99 with (mean 93.47± 4.08) this indicates a clear improvement of cases.). In our study there was statistically insignificant decrease in External rotation which reduced by 6o at adduction and by 5o at 90o of abduction.<br/>Conclusions:<br/>Arthroscopic inferior capsular shift procedure is safe and has good short-term outcomes with a low recurrence rate at 1-year minimum follow-up. |

| Summary, etc. | إزاحة الكبسولة السفلية بالمنظار العلاج الأمثل لعدم استقرار الكتف الأمامي السفلي في حال عدم وجود فقدان في عظام الشفة أو الحُقّ أو العضد. تتيح هذه التقنية فحصًا تشخيصيًا للكتف، وتحضير عظم الحُقّ، وتثبيتًا آمنًا وتشريحيًا للعقدة الكبسولية الحُقّية باستخدام الخطاطيف ، وتجنب إصابة الهياكل الطبيعية. <br/>يشترط للإصلاح بالمنظار جودة كافية لنسيج الكبسولة الحُقّية لتثبيت الخطاطيف ، ومريضًا متحمسًا يلتزم ببروتوكولات إعادة التأهيل بعد الجراحة.<br/>يتم وضع الخطاف في موضعه عند الساعة 4:30، تُمرّر خيطة عبر نسيج الكبسولة، ويُجرى ثني الكبسولة باستخدام جهاز نقل الخياطة عند الساعة 5:30. تُوفّر تقنية الثني والضغط هذه درجة أكبر من ثني الكبسولة من خلال تكوين طية من النسيج عند الشد. يمكن أخذ خيط جراحي فردي آخر. من المهم إجراء الطي من الأسفل إلى الأعلى على طول الحُقّ للحفاظ على وضوح الطي والتحكم فيه. يُستخدم مثبت مرساة لتمريره عبر الكبسولة إلى الشفة الحُقّية للطي. يتناسب انخفاض حجم الكبسولة طرديًا مع كمية وحجم الطي. |
