| Summary, etc. | Background: Infected femoral artery pseudoaneurysms (IFAPs) due to parenteral drug injection pose significant challenges in vascular surgery, with high risks of limb loss and mortality. This study aimed to identify predictors of limb salvage after ligation of IFAPs in drug abusers and develop a prognostic scoring system for amputation risk.<br/>Methods: A prospective cohort study was conducted on 250 patients presenting with infected femoral pseudoaneurysms at Kasr-Al Aini Hospital from April 2023 to April 2024. Patients underwent ligation with or without revascularization. Preoperative factors including ( hemorrhagic and septic parameters, intact pedal pulses, evidence of deep vein thrombosis), intraoperative factors including ( sever necrotizing infection, level of ligation, post clamping stump pressure of superficial femoral and profunda arteries, oxygen saturation of toes, peak systolic velocities of pedal arteries, immediate ankle brachial index post ligation) and postoperative factors including (patient condition ,acidosis, hyperkalemia, limb condition, poor capillary refill, tender and tense calf muscles, motor power ,sensory function ) were analyzed to determine their association with limb salvage. Statistical methods included logistic regression and ROC curve analysis to develop a predictive scoring system.<br/>Results: At one-month follow-up, the limb salvage rate stood at 60.4%, which progressively decreased to 30.4% by the 12-month assessment, Key predictors of amputation included severe necrotizing infection (OR = 5.5, p < 0.001), secondary hemorrhage post-revascularization (OR = 15.4, p < 0.001), proximal iliac ligation (OR = 5.8, p = 0.045), sepsis (OR = 1.8, p = 0.039), and immediate postoperative ankle-brachial index (ABI) > 0.425 (OR = 2.15, p = 0.004). A prognostic scoring system incorporating these factors demonstrated strong predictive ability (AUC = 0.855, 95% CI: 0.783–0.888), with a cutoff score of -0.5 balancing sensitivity (84.8%) and specificity (73.5%).<br/>Conclusion: Femoral artery ligation is a viable treatment for infected femoral pseudoaneurysm, but limb salvage depends on careful perioperative and intraoperative assessment. Severe necrotizing infection, secondary hemorrhage, proximal ligation of common femoral artery, graft occlusion or graft ligation after secondary hemorrhage post revascularization are critical predictors of amputation. The developed scoring system aids in early risk stratification, guiding clinical decisions to optimize outcomes. Further validation in larger cohorts is recommended to enhance its utility. |

| Summary, etc. | هدفت هذه الدراسة المستقبلية إلى تقييم عوامل التنبؤ لإنقاذ الأطراف لدى 250 مريضاً من متعاطي المخدرات عن طريق الحقن الوريدي والذين يعانون من تمدد شرياني كاذب معدٍ في الشريان الفخذي، وذلك في مستشفى القصر العيني بين أبريل 2023 وأبريل 2024. أظهرت النتائج أن ربط الشريان الفخذي مع أو بدون إعادة توعية يظل خياراً علاجياً فعالاً، حيث تم الحفاظ على الطرف بنجاح في 60.4% من الحالات. ومع ذلك، كشفت الدراسة أن ما يقرب من 40% من المرضى احتاجوا في النهاية إلى بتر كامل، مما يؤكد على خطورة هذه الحال.<br/>حددت الدراسة عدة عوامل تنبؤية رئيسية لسوء النتائج، حيث ظهرت العدوى الناخرة الشديدة كأهم عامل خطر لفقدان الطرف، مما يزيد احتمالية البتر بأكثر من خمسة أضعاف. بينما أظهر النزف الثانوي بعد عمليات إعادة التوعية ارتباطاً أكبر، حيث زاد خطر البتر بخمسة عشر ضعفاً. كما أثبت مستوى ربط الوعاء الدموي أهميته، حيث أظهر ربط الشريان الحرقفي القريب نتائج أسوأ بكثير مقارنةً بالربط عند مستويات أكثر طرفية. وشملت العوامل التنبؤية الإضافية المهمة وجود إنتان جهوي وضعف التروية كما تم قياسه بواسطة مؤشر الكاحل العضدي بعد الجراحة.<br/>تمثل المساهمة الرئيسية لهذا العمل في تطوير نظام تنبؤي جديد يصنف المرضى حسب خطر البتر، حيث حقق دقة تنبؤية ممتازة بمنحنى ROC مساحته 0.855. تتضمن هذه الأداة متغيرات ما قبل الجراحة وأثناءها لتوجيه القرارات السريرية، خاصة في تحديد الحالات التي تستدعي إعادة التوعية رغم وجود مجال جراحي معدٍ.<br/>تؤكد النتائج على أهمية الإجراءات العدوانية لمكافحة العدوى، والاختيار الدقيق لمستوى الربط، والمراقبة الدقيقة بعد الجراحة لدى هذه الفئة عالية الخطورة. بينما تقدم الدراسة رؤى قيمة لإدارة هذه الحالات المعقدة، يوصي المؤلفون بإجراء المزيد من الدراسات متعددة المراكز لتحسين النموذج التنبؤي وفهم النتائج الوظيفية طويلة المدى بعد إجراءات إنقاذ الأطراف. تمثل هذه الأبحاث خطوة مهمة نحو الإدارة القائمة على الأدلة لهذه المضاعفات المدمرة لإدمان للمخدرات عن طريق الحقن الوريدي. |
