| Summary, etc. | Background: The peroneus longus was a long muscle located superficially in the lateral compartment of the leg, along with the peroneus brevis muscle. The peroneus longus originated from the proximal aspect of the fibula and inserted into the medial cuneiform and the base of the first metatarsal bone. Its primary functions were to provide first ray plantarflexion and foot eversion.<br/> Aim and objectives: to evaluate the extent of donor site morbidity and assess ankle strength following the harvest of the ipsilateral peroneus longus tendon for ACL reconstruction in patients with primary ACL injuries.<br/> Subjects and methods: This descriptive study was conducted on 58 patients diagnosed with ACL injuries at El Qasr Al Ainy Hospital and Al Haram Specialized Hospital. <br/>Result: there was highly statistically significant difference at different times as regarded FADI score, AOFAS score, and CPEI score.<br/> Conclusion: The use of the peroneus longus tendon (PLT) as an autograft for ACL reconstruction demonstrates favorable clinical outcomes with minimal donor site morbidity. Functional scores such as FADI, AOFAS, and CPEI in our study showed significant improvement by the 6-month postoperative mark, indicating effective recovery of both knee and donor ankle functions. |

| Summary, etc. | العضلة الشظوية الطويلة ، والمعروفة أيضًا باسم العضلة الليفية الطويلة ، هي عضلة طويلة تقع بشكل سطحي في الحجرة الجانبية للساق، إلى جانب العضلة الشظوية القصيرة. تنشأ العضلة الشظوية الطويلة من الجزء القريب من عظم الشظية (الفيبولا)، وتُدرج في العظم الوتدي الإنسي وقاعدة المشط الأول.<br/>تتمثل وظائفها الأساسية في إنزال الشعاع الأول وبسط القدم للخارج.<br/>في السنوات الأخيرة، استخدم جراحو العظام وتر العضلة الشظوية الطويلة كرقعة ذاتية في إعادة بناء الرباط الصليبي الأمامي ، بالإضافة إلى الأوتار المأخوذة من أوتار العضلات المأبضيةأو وتر الرضفة مع العظم. كما تم استخدام هذا الوتر في ترميم الرباط الدالي ورباط الرضفة الإنسي نظرًا لتأثيره التآزري مع العضلة الشظوية القصيرة، مما يوفر فائدة وظيفية محتملة عند إجراء تثبيت لكليهما.<br/>من أبرز مزايا هذا الوتر قدرته على التجدد، كما أظهرت عدة دراسات باستخدام التصوير بالرنين المغناطيسي والتي بينت تجدد الوتر خلال عام واحد من استخراجه.<br/>علاوة على ذلك، أُشير إلى أن وتر العضلة الشظوية الطويلة قد يكون متفوقًا على وتر العضلة المأبضية في عمليات إعادة بناء الرباط الصليبي الأمامي ذات الحزمة الواحدة ، بسبب قطره الأكبر وانخفاض معدل الضمور في موقع المتبرع.<br/>ومع ذلك، لا تزال هناك بعض المخاوف بخصوص العجز الوظيفي المحتمل بعد أخذ وتر الشظوية الطويلة، لا سيما ما يتعلق بقوة بسط القدم للخارج وقدرة إنزال الشعاع الأول. ورغم أن العديد من الدراسات السريرية أفادت بحدوث انخفاض ضئيل في الوظيفة في موقع التبرع على المدى القصير، إلا أن التأثير طويل الأمد لاستخدام وتر العضلة الشظوية الطويلة على ميكانيكا القدم والكاحل لا يزال غير واضح، مما يبرز الحاجة إلى المزيد من الأبحاث لتقييم النتائج الهيكلية والوظيفية لاستخدام هذا الوتر كطعوم ذاتية. |
