| Summary, etc. | Background: Stroke is characterized by the sudden onset of clinical signs indicating focal or global disruption of cerebral function, with symptoms persisting for at least 24 hours or resulting in death.Purpose: To compare betweenthe effect of Action Observation Training versus Task Oriented Training on reaching and hand function in patients with Chronic Stroke. Methods: Thirty Ninemale chronic strokepatients, aged from 50 to 65 years were included in the study. They were randomly assigned into three equal groups: Group “A” was the control group, Group “B” was the study group I and Group “C” was the study group II). Patients in the control group received a selected physical therapy program for upper limb for 60 minutes, patients in group B received Task Oriented Training for upper limb for 30 minutes and the same selected physical Therapy program for upper limb for 30 minutes, while patients in group C received Action Observation Training for upper limb for 30 minutes and the same selected Physical Therapy Program for upper limb for 30 minutes. Treatment was conducted three times per week for four consecutive weeks. Reaching and Hand Function measurements were assessed using Reaching Performance Scale for stroke (RPSS), WolF Motor Function Test (WMFT) and hand held dynamometer (HHD) pre and post treatment.Results:There was a significant increase in hand grip strength and scores of wolf motor function test and reaching performance scale in the three groupspost treatment (p < 0.05). Moreover, there was a significant difference between the three groups post treatment in all measured outcomes (p < 0.05).Pairwise analysis revealed significant increase hand grip strength, wolf motor function test, reaching performance scale close target & reaching performance scale far target in group C compared to groups A & B. Also, there was a significant difference between groupsA and B in favor of group B.Conclusion: Action Observation Training is more beneficial than Task Oriented Training on reaching andhand function in patients with chronic stroke. |

| Summary, etc. | مرجعية: السكتة الدماغية هي علامات سريرية سريعة التطور لاضطراب بؤري (أو شامل) في وظائف الدماغ، مع استمرار الأعراض لمدة أربعة و عشرون ساعة أو أكثر، أو قد تؤدي إلى الوفاة. غالبًا ما يظهر ضعف اليد في المرحلة المزمنة بعد السكتة الدماغية، ويتجلى غالبًا في انخفاض قوة الأصابع واختلال في ثني اليد. يُعدّ الوصول إلى اليد أمرًا ضروريًا للأنشطة اليومية مثل الشرب، واستخدام شاشة اللمس، أو تشغيل أزرار المصعد.الغرض: دراسة فعالية التدريب بمراقبة الفعلضدالتدريب نحو المهام في تحسين الوصول ووظائف اليد لدى مرضى السكتة الدماغية المزمنة. طرق البحث: أُجريت الدراسة على تسعة وثلاثين ذكراً مريضًا بالسكتة الدماغية المزمنة، تراوحت أعمارهم بين خمسونوخمسة و ستونعامًا، تم توُزيّعهم عشوائيًا إلى ثلاث مجموعات متساوية (المجموعة "أ" (المجموعة الضابطة)، والمجموعة "ب" (مجموعة الدراسة 1) والمجموعة "ج" (مجموعة الدراسة 2). تلقى مرضى المجموعة الضابطة برنامجًا علاجا طبيعيًا محددًا للطرف العلوي لمدة ستون دقيقة، وتلقى مرضى المجموعة (ب) تدريبًا نحو المهام للطرف العلوي لمدة ثلاثون دقيقة مع نفس برنامج العلاج الطبيعي المحدد للطرف العلوي لمدة ثلاثون دقيقة, بينما تلقى مرضى المجموعة (ج) تدريبًا على مراقبة الفعل للطرف العلوي لمدة ثلاثون دقيقة مع نفس برنامج العلاج الطبيعي المحدد للطرف العلوي لمدة ثلاثون دقيقة. أُجري العلاج ثلاث مرات أسبوعيًا لمدة أربعة أسابيع متتالية.تم تقييم قياسات الوصول ووظيفة اليد باستخدام مقياس القوة المحمول باليد، ومقياس أداء الوصول لمرضي السكتة الدماغية، وإختبار وولفلوظائف الحركة، ومقارنتها لجميع المجموعات قبل و بعد العلاج.النتائج:أظهرت نتائج ما بعد العلاج وجود زيادة ذات دلالة إحصائية في المجموعات الثلاث في قياسات قوة قبضة اليد، وإختبار وولف لوظائف الحركة، ومقياس أداء الوصول لمرضي السكتة الدماغية للهدف القريب و الهدف البعيد. كما أظهرت النتائج بعد العلاج وجود فارق إحصائي بين المجموعات الثلاث في قوة قبضة اليد، ومتوسط درجات إختبار وولف لوظائف الحركة، ومقياس أداء الوصول لمرضيالسكتة الدماغية للهدف القريب و الهدف البعيد حيث كان التحسن أكبر في المجموعة (ج) مقارنةً بالمجموعتين (أ) و(ب)كذلك في المجموعة (ب) مقارنةً بالمجموعة (أ). |
