| Summary, etc. | he incidence of urolithiasis in urology is about 1-5% with a recurrence rate of <br/>the disease about 50% (Alatab et al., 2016; Abeywickarama e al., 2016). As <br/>proven by studies urolithiasis has been constantly increasing every year (Modi <br/>et al., 2018). One of the most frequent types of urinary calculi are upper ureteric <br/>stones with a common clinical picture of renal colic and hematuria (Yasui et <br/>al., 2013). <br/>The treatment of Proximal Ureteral Stones (PUS) is mainly targeting <br/>provision of high Stone-Free Rate (SFR) with reduction of associated <br/>morbidities. The evolution in the endourological techniques led to the <br/>appearance of numerous new modalities in treatment of PUS. Those modalities <br/>include Extracorporeal Shock Wave Lithotripsy (ESWL), Ureteroscopy (URS) <br/>(semirigid-flexible, retrograde-ante grade), Percutaneous Nephrolithotomy <br/>(PCNL), laparoscopic ureterolithotomy, and open surgery (Preminger, 2007; <br/>Demirbas et al., 2016). <br/>Flexible ureteroscopy (fURS) procedure has numerous advantages. It is <br/>minimally invasive with lower risk of developing complications. Also, it is <br/>beneficial for either patients who have complex anatomical kidneys or patients <br/>having bleeding diathesis or those on anticoagulants (Prabhakar, 2010). <br/>FURS is widely rendered the treatment of choice in management of <br/>upper urinary tract stones (Zeng et al., 2022). For intra-renal or proximal <br/>ureteric stones <20 mm FUS and shockwave lithotripsy (SWL) are considered <br/>the primary treatment modalities (Hyams et al., 2015). However, studies <br/>showed that a higher single procedure success rate and a lower second re-<br/>surgery rate favored fURS as compared to SWL (Zhang et al., 2015). |

| Summary, etc. | هدفت هذه الدراسة إلى تقييم ومقارنة فعالية وأمان منظار الحالب المرن ومنظار الحالب شبه الصلب (في علاج حصوات الحالب القريبة التي لا تتجاوز 2 سم. وكان الهدف الأساسي هو تحديد معدل خلو الحصوات ومعدلات المضاعفات ونتائج العمليات لتوجيه اختيار الطريقة الأمثل للعلاج.<br/> أُجريت دراسة عشوائية مستقبلية محكمة في مستشفيات جامعة القاهرة خلال الفترة من يوليو 2022 حتى يوليو 2024. <br/>شملت الدراسة 130 مريضًا تتراوح أعمارهم بين 15 و65 عامًا ولديهم حصوة منفردة في الحالب القريب، وتم توزيعهم عشوائيًا إلى مجموعتين متساويتين: المجموعة (أ) خضعت للتفتيت باستخدام المنظار المرن، والمجموعة (ب) باستخدام المنظار شبه الصلب. خضع جميع المرضى لتقييم ما قبل العملية، وتم إجراء التفتيت بالليزر تحت التخدير.<br/>تم تقييم النتائج من حيث زمن العملية، مدة الإقامة بالمستشفى، المضاعفات (حسب تصنيف Clavien–Dindo)، ومعدل خلو الحصوات بعد 4 أسابيع. أظهرت النتائج تقارب المجموعتين من حيث الخصائص الديموغرافية وصفات الحصوة. حقق المنظار المرن معدل خلو حصوات أعلى (89.2%) مقارنة بالمنظار شبه الصلب (76.9%)، مع قدرة أفضل على الوصول للحصوات خاصة في الحالات ذات الصعوبات التشريحية، لكنه استغرق وقتًا أطول في العملية (54.3 مقابل 41.7 دقيقة). كانت مدة الإقامة ومعدل المضاعفات متشابهة بين المجموعتين وغالبية المضاعفات بسيطة. خلصت الدراسة إلى أن الطريقتين آمنتان وفعالتان، حيث يوفر المنظار المرن نسب نجاح أعلى ومرونة أكبر، بينما يظل المنظار شبه الصلب خيارًا عمليًا وأقل تكلفة في حال عدم توفر المناظير المرنة. |
