| Summary, etc. | Background: Rheumatoid arthritis (RA) is a chronic inflammatory disease treated with methotrexate (MTX). Despite its efficacy, long-term MTX use may lead to nephrotoxicity and renal fibrosis..<br/>Aim of work: The aim of our study is to assess the role of renal parenchymal shear wave elastography in detecting early signs of nephrotoxicity in patients taking methotrexate.<br/>Methods: 20 patients diagnosed with RA and treated with methotrexate for at least 6 months who were compared with 20 healthy controls were enrolled in our study.Shear wave elastography assessment given as kilopascals (kPa), was measured. This study was conducted from December 2024 to June 2025.<br/>Results: SWE demonstrated significantly increased renal stiffness in RA patients compared to controls across all renal regions. The left kidney showed a more uniform stiffness increase, while the right kidney exhibited pronounced changes at the upper pole. Total kidney SWE provided the highest diagnostic accuracy, with left kidney SWE achieving 95% sensitivity, specificity, and predictive values at a cutoff >19.9 kPa.<br/>Conclusion: Shear wave elastography (SWE)proved to be a sensitive, non-invasive tool for detecting early methotrexate-induced renal changes, outperforming conventional ultrasound and standard renal function tests. Its incorporation into routine monitoring of RA patients may enable earlier intervention and improved renal outcomes. |
