| Summary, etc. | Background: Expanding MRI screening has clear diagnostic advantages, but conventional breast MRI is lengthy (45–60 minutes) generating large number of images to be interpretated. To achieve broad, feasible, and affordable screening, protocols must be faster, simpler, and easier to interpret. Abbreviated breast MRI (abMRI) is a streamlined approach designed to answer focused clinical questions while reducing scan and reporting time. Studies showed that abMRI maintains diagnostic accuracy comparable to full-protocol MRI, supporting its role as a potential screening tool for women with dense breasts. Protocols vary across institutions but typically include a single post-contrast sequence and fewer non-contrast sequences, achieving efficiency while preserving tumor detection and recall rates. We aimed to explore the diagnostic efficacy, clinical utility, and radiological characteristics of abbreviated breast MRI protocols in comparison with conventional dynamic contrast-enhanced MRI.<br/>Objective: This thesis aimed to explore the diagnostic efficacy, clinical utility, and radiological characteristics of abbreviated breast MRI protocols in comparison with conventional DCE-MRI.<br/>Methods: This prospective, cross-sectional analytical study included 93 female patients (aged 21–82 years) and the patients underwent breast MRI either for screening or diagnosis. Imaging was performed using a 1.5T Philips Achieva scanner. A full dynamic contrast-enhanced MRI protocol was acquired, from which two abbreviated protocols (FAST and Abridged) were derived. All studies were independently reviewed by two expert radiologists, blinded to clinical data. Lesions were evaluated according to BI-RADS criteria, with histopathological confirmation in 76 lesions and imaging follow-up for others.<br/>Results: This study was conducted on 93 female patients with a total of 186 breast sides assessed revealing 206 individual findings. The most common indication was diagnostic evaluation (66.7%), and the predominant breast density was heterogeneous fibroglandular tissue (45.2%). Mass lesions were the most frequently detected finding (30.6%), and dynamic MRI and abridged protocols showed identical lesion detection, while the fast protocol missed cysts and some benign lesions. High agreement was observed between the Dynamic and Abridged Abbreviated protocols across lesion BI-RADS classification (κ = 1.000, p < 0.001). The Fast protocol showed slightly lower agreement (κ = 0.913). Out of 76 biopsied lesions, 65 were malignant. The Dynamic and Abridged protocols had a sensitivity of 95.4% and specificity of 96.5% while the Fast protocol showed equal sensitivity, 95.4% but lower specificity, 95.7%.<br/>Conclusion: Abbreviated breast MRI is a promising, efficient, and diagnostically reliable alternative to full-protocol breast MRI for screening women at intermediate to high risk. Its integration into clinical practice could improve screening accessibility, reduce costs, and maintain early detection rates without compromising diagnostic accuracy. |

| Summary, etc. | أُجريت هذه الدراسة المقطعية التحليلية في كلية طب قصر العيني وشملت 93 مريضة أُحلن من قسم الجراحة ومن البرنامج المصري للكشف المبكر عن سرطان الثدي لأغراض تشخيصية أو للفحص الدوري. خضعت جميع المريضات لتصوير بالرنين المغناطيسي للثدي بعد إجراء السونو–ماموجرام، وذلك باستخدام جهاز رنين مغناطيسي بقوة 1.5 تسلا وملف مخصص ثنائي للثدي. تضمنت البروتوكولات القياسية للتصوير تسلسلات T1WI و T2WI، والتصوير الديناميكي المعزز بالمادة الكاشفة مع صور الطرح، بالإضافة إلى التصوير بالانتشار (DWI) مع خريطة معامل الانتشار (ADC). تم الحصول على الارتباط النسيجي المرضي للبؤر المشبوهة عن طريق الخزعة الموجهة بالموجات فوق الصوتية أو الاستئصال الجراحي . تم تقييم بروتوكولين مختصرين للتصوير بالرنين المغناطيسي: بروتوكول FAST (يقتصر على صور T1 قبل الحقن وبعد الدقيقة الثانية من الحقن بالمادة الكاشفة).بروتوكول المختصر الموسع، الذي شمل بالإضافة إلى ذلك تسلسلات T2 و DWI بهدف تحسين توصيف الآفات.<br/>كان النسيج الغدي الليفي الكثيف غير المتجانس (45.2%) والمنتشر بشكل متفرق (37.6%) هما الأكثر شيوعًا بين العينة المدروسة. وكانت أنماط التعزيز الخلفي للنسيج الغدي الخفيف والمتوسط هي الأكثر ملاحظة، حيث ظهرت في 46.2% و37.6% من الحالات على التوالي. ارتبطت الأورام الخبيثة بشكل أكبر بزيادة كثافة النسيج، بينما لم يُسجَّل أي ورم خبيث لدى المريضات ذوات النسيج الدهني.<br/>أظهرت معدلات كشف الآفات تقاربًا كبيرًا بين البروتوكولات. وكانت الكتل هي الاكتشاف الأكثر شيوعًا (30.6%)، تلتها مناطق التعزيز غير الكتلي (9.7%). تطابق البروتوكول المختصر الموسع مع البروتوكول الديناميكي (DCE-MRI) في كشف مختلف أنماط الآفات، بينما أظهر بروتوكول FAST أداءً أقل قليلًا في تحديد بعض الكيانات الحميدة (مثل الأكياس والتنخر الدهني) لغياب تسلسلات T2WI و DWI.<br/>تمت ملاحظة اتفاق عالٍ بين البروتوكولات فيما يتعلق بشكل الآفة، وأنماط التعزيز، وعمق الآفة. وكان التوزيع القطاعي هو النمط الغالب بين حالات التعزيز غير الكتلي، كما أظهرت قياسات حجم الآفة تطابقًا ممتازًا. |
