| Summary, etc. | Aim: The aim of the present study was to evaluate treatment of aberrant frenum attachment <br/>using frenotomy combined with free gingival graft (FGG) with respect to the gain in the width <br/>of keratinized gingiva, relapse, mucosal healing and postoperative pain as well as scar <br/>formation. <br/>Methodology: Twelve patients with aberrant mandibular frenum attachment underwent <br/>frenotomy combined with free gingival grafting at the Faculty of Dentistry, Cairo University, <br/>and October University for Modern Sciences and Arts. Frenotomy was performed using split- <br/>thickness incisions on both sides of the frenum, followed by apical relocation of the muscle <br/>through blunt dissection. A palatal free gingival graft was then harvested and secured to the <br/>recipient site with resorbable sutures. <br/>Results: Keratinized tissue increased from 2.08 mm to 8.83 mm at 3 months (p<0.001). Pain <br/>scores dropped from 5.83 on Day 1 to 0.33 on Day 7 (p<0.001). IPR index showed successful <br/>healing with scores of 5.33 (inflammatory), 4.00 (proliferative), and 2.17 (remodelling), with a <br/>total of 11.5 ± 0.45 indicating excellent healing in 83.3% of cases (p=0.021). Mean scar index <br/>at 3 months was 4.17 ± 0.94, reflecting mild to moderate scarring. <br/>Conclusion: This combined approach is an effective, safe, and predictable modality for treating <br/>aberrant labial frenum attachments in adults, particularly where long-term periodontal health <br/>and soft tissue augmentation are the primary goals. |

| Summary, etc. | يُعد اللجام الشفوي غير الطبيعي، خصوصا˝ في المنطقة الأمامية للفك السفلي، من المشاكل الشائعة في اللثة والأنسجة الفموية، حيث يؤدي إلى قلة عمق الدهليز الفموي وصعوبة في تنظيف الاسنان ، كما قد يسبب انحسار اللثة ويساهم في عودة الفرجة بين القواطع الوسطى بعد إغلاقها تقويميا.˝ لذلك يُعتبر التدخل الجراحي ضروريا˝ للحفاظ على صحة اللثة والأنسجة الداعمة.<br/>وقد وُجدت عدة طرق جراحية لعلاج هذه المشكلة مثل استئصال اللجام، قطع اللجام، وكذلك تقنيات أخرى, إلا أن هذه الطرق قد تُسبب عودة الارتباط أو نقصا˝ في كمية اللثة المتقرنة أو ظهور ندبات. ويُعد الجمع بين قطع اللجام وزراعة طُعم لثوي حر من أكثر الطرق نجاحا˝، حيث يحقق تصحيحا˝ وظيفيا˝ مع زيادة عرض اللثة المتقرنة مما يضمن استقرار النتائج على المدى الطويل.<br/>شملت هذه الدراسة السريرية اثني عشر مريضا˝ بالغا˝ لديهم لجام شفوي سفلي غير طبيعي. أُجري قطع اللجام باستخدام شقوق نصف سمكية مع إعادة توجيه الألياف بعيدا˝ إلى الأسفل، ثم زُرع طُعم لثوي مأخوذ من الحنك وثُبّت في مكانه بخيوط قابلة للامتصاص . جرى تقييم المرضى من حيث التغير في عرض اللثة المتقرنة، وشدة الألم بعد العملية، وسرعة الالتئام، وظهور الندبات بعد ثلاثة أشهر.<br/>أظهرت النتائج زيادة ملحوظة في عرض اللثة المتقرنة من ٢.٠٨ ملم قبل الجراحة إلى ٨.٨٣ ملم بعد ثلاثة أشهر، مع انخفاض واضع في الألم من ٥.٨٣ في اليوم الأول إلى ٠.٣٣ في اليوم السابع. كما أظهر معظم المرضعى التئاما˝ ممتازا˝ بنسبة ٨٣.٣٪، بينما كانت الندبات خفيفة إلى متوسطة بمتوسط ٤.١٧ بعد ثلاثة أشهر، دون تأثير جمالي أو وظيفي يُذكر.<br/>وبنا˝ء على ذلك، يمكن القول إن الجمع بين قطع اللجعام والطُعم اللثوي الحر يُعد أسلوبا˝ آمنا˝ وفعالا˝ يمكن التنبؤ<br/>بنتائجه، حيث يُزيل الشد غير المرغوب فيه ويُوفر شريطا˝ مستقرا˝ من اللثة المتقرنة، مما يُحسن صحة اللثة على المدى الطويل ويُعطي نتائج جمالية مرضية. |
