| Summary, etc. | Introduction: Gastroschisis (GS) is a congenital anomaly that requires surgical repair, often resulting in significant morbidity and mortality. Postoperative feeding protocols play a crucial role in determining outcomes. we implemented a standardized feeding protocol for infants post-gastroschisis repair at our institution<br/>Objectives: To initiate and establish a Standardized feeding protocol that involves gradual introduction of enteral feeds, with close monitoring of tolerance and adjustment of feeding volumes and rates as needed, which improve feeding tolerance, reduce TPN use, and shorten length of stay.<br/><br/>Method: we prospectively reviewed the outcomes of infants who underwent gastroschisis repair and received the standardized feeding protocol.<br/><br/>Results: 25 cases of GS were included, Simple GS were 21/25 cases (84%) with a mortality of 14/21 (66%), while complex GS were 4/25 cases (16%) with a mortality of 3/4 (75%). Eight cases (32%) were discharged from the unit alive, while seventeen cases (68%) died.<br/>Conclusion: A standardized feeding protocol minimized variability in practice, facilitated earlier achievement of full enteral feeding, and reduced the risks associated with prolonged parenteral nutrition. Importantly, the high level of nurse satisfaction reflected the simplicity, clarity, and safety of the protocol in daily practice. |

| Summary, etc. | تمثل حالات الإنشقاق الخلقي بجدار البطن الأمامي تحديا معقدا علي مستوي الجراحة والتغذية في رعاية حديثي الولادة خاصة في الدول محدودة ومتوسطة الدخل والتي تعيق مواردها المحدودة الحالات الإكلينيكية المعقدة . بعد الاصلاح الجراحي لهذة الحالات يعتبر عدم استجابة الحالات للتغذية الفمويه والاعتماد المطول علي التغذية الوريدية سبب رئيسي لطول فترة الإقامة بالمستشفي مما استلزم وجود دراسة قائمة علي الدليل لمناجزة التغذية لهذه الحالات .<br/>وقد قيمت هذة الدراسة المستقبلية الجماعية فاعلية وتأثير بروتوكول التغذية المنظم لعدد خمسة وعشرون حالة تعاني من الإنشقاق الخلقي بجدار البطن الأمامي في مستشفي الأطفال التخصصي بجامعة القاهرة علي مدار خمسة عشر شهرا . وأبرز هذا البروتوكول عدة نقاط , إدخال تدريجي للتغذية عن طريق الفم , تفضيل لبن الأم , زياده ممنهجه للتغذية , التعاون بين كافة التخصصات للفريق الطبي .<br/><br/>أوضحت النتائج الأولية نجاح تطبيق البروتوكول بمستوي إرضاء عالي بين فرق التمريض بنسبة (87.5%) وأقروا بسهولة تطبيقة مما يدل علي جدواه عمليا وقبولة من الطاقم الطبي .<br/>أوضحت النتائج الثانويه بأن متوسط عدد الأيام اللازمه لبدء التغذية بعد الجراحة كانت 11.75 يوم وعدد الأيام اللازمة للوصول التغذية الكاملة عن طريق الفم كانت 10.38 يوم بين الناجين مما يمثل أفضلية مقارنة بالنتائج المنشورة والتي أقرت فترة 14.21 يوم لبدء التغذية .<br/>خلصت الدراسة إلي أن وجود توصيات منظمة للتغذية تساعد علي وجود استمرارية من الناحية العملية وتساعد علي الزيادة الممنهجة في التغذية وتؤزر كفائة العمل الجماعي . أوضح هذا البروتوكول معاير واضحه لاتخاذ القرار وأكد علي متابعة الإستجابة للتغذيه واعطاء نمط يمكن تطبيقة لمناجزة تحديات التغذية المصاحبة لرعاية حالات الإنشقاق الخلقي بجدار البطن الأمامي .<br/>وبالرغم من أن النسبة العامة للناجين تمثل 32% في هذة الإمكانيات المحدوده تظل هذه النتائج أقل بكثير من متوسط النسب في الاوساط ذات الدخل العالي حيث تقترب نسبة الناجيين من 100% . أكدت هذة النتائج علي أن بروتوكول التغذيه يمثل أداه قيمة لتحسين النتائج ولكن الرعاية الشاملة لهذه الحالات تتطلب وجود عوامل رئيسية أخري أوسع وأشمل مثل توقيت نقل الحالات , تطبيق سياسات مكافحة العدوي, والتنسيق بين الفرق الطبيه المختلفة .<br/>يوضح هذا البحث أن توصيات التغذيه القائمه علي الدليل تمثل تدخل يمكن تطبيقه مما يؤدي إلي تحسين مستوي الخدمة وبالتالي تقليل فترة التغذية وتحسين مستوي الرضا لدي الفريق الطبي .<br/>هذه النتائج تدعم التطويرالمستمر وتطبيق بروتوكولات تغذيه منظمه كعنصر أساسي في المناجزه الشامله لهذه الحالات في الأوساط محدوده الدخل. |
