| Summary, etc. | Aim: To compare the effect of a final irrigation with antimicrobial - corticosteroid mixture <br/>with cryotherapy on post-instrumentation pain and microbial reduction in patients with single canal <br/>teeth with necrotic pulps and asymptomatic apical periodontitis. <br/>Methodology: Twenty-eight patients having single canal teeth diagnosed with necrotic pulp <br/>and asymptomatic apical periodontitis were recruited to participate in the study. Root canal <br/>treatment was performed in two visits. At the first visit, access cavity was prepared, preoperative <br/>microbial samples (S1) were collected, mechanical preparation was performed using Hyflex CM <br/>files with 2.5% NaOCl irrigation, then post-instrumentation microbial samples (S2) were collected. <br/>Patients were then randomly allocated according to the final irrigation either to the “Mix group” <br/>receiving a mixture of levofloxacin, dexamethasone and fluconazole or to the “Cryo group” <br/>receiving cryotherapy, then the third microbial samples (S3) were collected. Patients were requested <br/>to record their pain intensity on a 10-point numerical rating scale (NRS) at 6,12, 24, 48 and 72 <br/>hours postoperatively. In the recall visit after 1 week, after confirming that patients were <br/>asymptomatic, canal irrigation was done with17% EDTA, saline and 2.5% NaOCl, then master cone <br/>fit and obturation were performed. <br/>Results: The two groups showed no significant increase in postoperative pain intensity at all <br/>time intervals compared to preoperative pain intensity. The two groups showed mild pain intensity <br/>at 6 and 12 hours postoperatively with no significant difference between the two groups. Moreover, <br/>no pain was reported in both groups at 24, 48 and 72 hours postoperatively. At the post-final <br/>irrigation microbial samples, both groups showed significant bacterial and candidal load reduction.<br/><br/><br/> <br/><br/>vii <br/><br/>Conclusion: There were no differences between the two groups regarding pain intensity, <br/>bacterial load and candidal load at all time intervals. In necrotic teeth with asymptomatic apical <br/>periodontitis, both the antimicrobial-corticosteroid mixture and the cryotherapy groups are suitable <br/>for use as a final irrigation after mechanical preparation in presence of 2.5% sodium hypochlorite to <br/>reduce postoperative pain and intracanal microbial loads. <br/>Clinical relevance: Teeth with necrotic pulps and asymptomatic apical periodontitis could <br/>be successfully treated with the use of the lower concentration (2.5%) NaOCl combined with either <br/>antimicrobial-corticosteroid mixture or cryotherapy with minimal postoperative pain intensity. This <br/>can help avoiding the hazards and complications of the full concentration 5.25% NaOCl. |
