| Summary, etc. | Aim: The aim of this study was to evaluate the effectiveness of the lingual appliance with lever Arm as compared to lingual appliance without lever arm during maxillary en -masse retraction regarding nasolabial angle changes, profile convexity reduction, lip retraction, torque control, anchorage loss, pain, speech difficulty, oral hygiene maintenance and eating difficulties. Methodology: 24 patients were randomized into 2 equal groups (n=24). One patient was dropped out from each group. In group 1 (intervention group) NiTi coil springs were attached to lever arms while in group 2 (Control group) the NiTi coil springs were attached to the canine brackets during maxillary en-masse retraction. The primary outcomes were improvement of the nasolabial angle and profile convexity reduction. The secondary outcomes were the differences in lip retraction, torque control, anchorage loss, pain, difficulty in speech, maintaining oral hygiene and eating. Outcomes were assessed using lateral cephalometric radiographs, Vas scale and likert scale. Results: No significant difference was found between the 2 groups regarding soft tissue changes, torque control, pain, maintaining oral hygiene or eating. Significant difference was found between both groups in maxillary central incisor root apex movement where it moved labially in intervention group (1.32 ± 1.38) and palatally in control group (-0.50 ± 1.88) respectively. Significant difference was found between both groups in anchorage, where the maxillary first molar moved mesially in intervention group (1.95±1.40) and distally in control group (-0.86±1.85) respectively. Significant difficulty in speech was found in the intervention group during the first day of retraction compared to control group. Conclusion: Lever arm mechanics was not able to prevent torque loss during maxillary en masse retraction. Lever arms also lead to more anchorage loss. |

| Summary, etc. | تهدف هذه الدراسة السريرية العشوائية الي تقييم تأثيراضافة أذرع الرفع الي جهاز تقويم الأسنان اللساني أثناء السحب الجماعي للأسنان الأمامية لدي المرضى الذين يحتاجون إلى خلع الضواحك العلوية الأولى.<br/> ضم البحث 24 مريضا وتم شرح الإجراء بالتفصيل مع فوائده المتوقعة وطلب منهم التوقيع على موافقة خطية مستنيرة. مريض واحد من كل مجموعة لم يستكمل الدراسة.<br/>بدأت الدراسة بتثبيت جهاز التقويم اللساني في كلا الفكين لكل مريض. تم تغيير اسلاك التقويم حتى تم الوصول إلى سلك نيكل تيتانيوم مقاس 0.017 × 0.025 بوصة. بالنسبة لحالات بروز الأسنان في الفكين، تم خلع الضواحك الأربعة الأولى وسحب الأسنان السفلية الأمامية قبل الفك العلوي بشهر لتجنب التلامس بين الأسنان الأمامية العلوية والسفلية اثناء السحب. بالنسبة لحالات العضة من النوع الثاني، تم البدء في سحب الأسنان العلوية مباشرة بعد اصطفافها و خلع الضواحك العلوية الأولي.<br/>أثناء سحب الأسنان، تم تثبيت دعامات تقويمية بين الأضراس العلوية الأولى والثانية من الجانب اللساني. وأعقب ذلك إدخال سلك علوي من الفولاذ المقاوم للصدأ مقاس 0.017 × 0.025 بوصة مخصص للتقويم اللساني. بالنسبة لمجموعة التدخل، تم لحام أذرع الرافعة بين القاطعة الجانبية العلوية والناب. تم تصنيع أذرع الرافعة من سلك معمل مقاس 0.9 مم وتم تعديل طولها لتكون علي مسافة 14 مم من سلك القوس الرئيسي. تم ربط نوابض لولبية من النيكل تيتانيوم بين أذرع الرفع والدعامات التقويمية. بالنسبة لمجموعة التحكم، تم ربط النوابض بين أقواس الأنياب والدعامات. تم ضبط القوة باستخدام مقياس القوة لتطبيق 150 جم لكل جانب أثناء سحب الأسنان. تم رؤية كل مريض شهريا لتعديل قوة السحب.<br/> طُلب من كل مريض ملئ استبيان للإبلاغ عن درجة الألم، حدوث مشاكل في الكلام و نطق الأحرف، وجود صعوبات في الحفاظ على نظافة الفم أو الأكل في نفس يوم بدء سحب الأسنان، بعد 24 ساعة، بعد أسبوع و بعد شهر. تم أخذ صور اشعة جانبية مباشرة قبل وبعد سحب الأسنان. |
