Summary, etc.	Purpose: The aim of this study is radiographic assessment of computer guided modified <br/>ridge splitting technique versus conventional free hand technique in management of <br/>horizontal difficiency in posterior mandible: a randomized control clinical trial. <br/>Methods: In this randomized controlled clinical trial, 18 patients with horizontally <br/>deficient posterior mandibular ridges were enrolled into the study and divided into two <br/>groups each group consisted of 9 patients . The first group ( study group ) , was <br/>augmented using guided modified ridge splitting grafting procedure, and the other group ( <br/>control group ) was augmented using conventional free hand modified ridge splitting <br/>grafting technique. This was followed by the radiographic superimposition of the <br/>preoperative CBCT scan with the immediate postoperative CBCT scan for each patient to <br/>the test for the accuracy of each technique. <br/>Results: The accuracy of the fixation guide was estimated by calculating the mean value <br/>of planned horizontal bone gain of the guided group (4.24 ±1.78) mm compared to the <br/>mean value of the actual immediate postoperative bone gain (4.5±1.5) mm, and the <br/>difference was not statistically significant (p=0.75). <br/>Conclusion: The use of patient specific surgical guide for the harvesting and fixation of <br/>the modified cortical shell in the horizontally deficient posterior mandible offered more <br/>accurate results than the freehand harvested and fixed modified cortical shell technique.

Summary, etc.	تعتبر عملية تكبير العظام الأفقية في الفك السفلي الخلفي من الإجراءات الصعبة للغاية، وذلك بسبب ضعف إمدادات الدم، وصعوبة الوصول إلى المنطقة، بالإضافة إلى محدودية تغطية الأنسجة الرخوة. وعلى الرغم من أن تطعيم العظام الذاتية هو أحد التقنيات الشائعة لتكبير النتوءات السنخية، إلا أنه يتطلب مهارات جراحية متخصصة، ووقتًا طويلاً أثناء العملية، إضافة إلى نقص التوجيه التشريحي عند تثبيت كتلة العظم، خاصة أثناء حفر البراغي الصغيرة، مما يعرض الهياكل الحيوية المجاورة للخطر. كما أن هناك خطرًا من وضع وتثبيت الكتلة العظمية بشكل غير دقيق، مما قد يؤدي إلى فشل عملية تطعيم العظام .<br/>كان هدف هذه الدراسة هو تقييم الكسب العظمي الافقي النهائي في الفك السفلي الخلفي ذو العجز الأفقى باستخدام البطانة الموجهة بالكمبيوتر مقارنة بتقنية التثبيت الحر باليد .<br/>تضمنت الدراسة 9 مرضى يعانون من فقدان عدة أسنان في الفك السفلي الخلفي مع حواف سنخية أفقية ناقصة على الجانبين. تم تكبير نصف الحواف السنخية باستخدام دليل جراحي مخصص للمريض، تم طباعته بتقنية ثلاثية الأبعاد لتثبيت العظم الذاتي المنشأ، بينما تم تكبير النصف الآخر باستخدام تقنية التثبيت الحر باليد مع دليل قطع مطبوع ثلاثي الأبعاد لحصاد كتل العظام في كلا المجموعتين .<br/>كان التئام الجروح طبيعيًا لدى جميع المرضى باستثناء مريض واحد عانى من التهاب في الجرح على جانب التحكم، وآخر عانى من تنميل في نفس الجانب، وقد زال التنميل بعد 4 أشهر. تم إجراء تصوير مقطعي محوسب (CBCT) بعد 4 أشهر من العملية لجميع المرضى، وأظهرت النتائج أن متوسط الكسب العظمي الأفقي بلغ (4.93±0.64) ملم، وهو أعلى بشكل إحصائي ملحوظ من متوسط الكسب في المجموعة الضابطة والذي بلغ (4.46±0.67) ملم .<br/>ضمن حدود هذه الدراسة، فإن استخدام دليل جراحي مخصص للمريض مطبوع بتقنية ثلاثية الأبعاد لحصاد وتثبيت العظام الذاتية في تكبير الحافة السنخية الأفقية للفك السفلي أدى إلى كسب عظمي أفقي أكبر مقارنة بتقنية التثبيت اليدوي، مع تقليل المضاعفات ما بعد الجراحة. وبالتالي، يعتبر هذا الأسلوب أكثر أمانًا ودقة ويوفر الوقت
