| Summary, etc. | Background: Mechanical neck pain is defined as pain originating from the anatomical structures in the cervical spine such as muscles, ligaments, facet joints, intervertebral discs and vertebrae without evidence of systemic disease or neurological involvement. Aim: The purpose of this study was to investigate which is more effective Instrument assisted soft tissue mobilization or Dry cupping on pain intensity, cervical range of motion, cervical proprioception and functional status in patients with chronic mechanical neck pain Design: Pre_post test single blind randomized control trial. Setting: The outpatient physiotherapy clinic of Helwan University hospital. Methods: Forty five Patients were randomly divided equally into three groups. Group (A)(study group):received instrument assisted soft tissue mobilization and conventional physical therapy.Group (B)(study group): received dry cupping and conventional physical therapy. Group (C)(control group): received conventional physical therapy (isometric strengthening exercises, stretching exercises, posture correction exercises and cold application).All the three group received three sessions per week for down weeks. The pre and post treatment development of the patients were evaluated by visual analogue scale(VAS) to measure pain, CROM device to measure cervical range of motion and cervical proprioception and Arabic neck disability index to measure functional status. Results: All three groups demonstrated progress in every research variable. Group(A) showed no statistically significant difference to group (B) regarding pain intensity and cervical range of motion, but group (A) showed more statistically significant difference than group (B) regarding cervical proprioception and functional status. Conclusion: Instrument-assisted soft tissue mobilization and dry cupping are both effective in alleviating pain and enhancing cervical range of motion; however, instrument-assisted soft tissue mobilization is superior to dry cupping in improving cervical proprioception and functional status in patients with chronic mechanical neck pain. |

| Summary, etc. | الخلفية: يُعرَّف الألم الميكانيكي في الرقبة بأنه ألم ينشأ من التراكيب التشريحية في العمود الفقري العنقي مثل العضلات، والأربطة، والمفاصل الوجيهية، والغضاريف بين الفقرية، والفقرات، وذلك دون وجود دليل على مرض جهازي أو اعتلال عصبي. وغالبًا ما يتركز في المنطقة العنقية وقد يزداد سوءًا مع حركات الرقبة أو الأوضاع الثابتة أو عند لمس الأنسجة المصابة.<br/>الهدف: الغرض من هذه الدراسة هو معرفة أيهما أكثر فاعلية: تحريك الأنسجة الرخوة باستخدام الاداه المساعده أم الحجامة الجافة في تقليل شدة الألم، وتحسين مدى حركة الرقبة، والإحساس الموضعي والحالة الوظيفية لدى المرضى الذين يعانون من الألم الميكانيكي المزمن في الرقبة.<br/>التصميم: تجربة عشوائية محكومة، مفردة التعمية، مع تقييم قبلي وبعدي.<br/>المكان: عيادة العلاج الطبيعي الخارجية بمستشفى جامعة حلوان.<br/>المنهجية: تم تقسيم المرضى عشوائيًا وبالتساوي إلى ثلاث مجموعات.المجموعة (أ) [مجموعة الدراسة]: تلقت التحريك اليدوى للأنسجة الرخوة بالإضافة إلى العلاج التقليدي (تمارين تقوية ساكنه ، تمارين استطاله ، تمارين تصحيح القوام، وكمادات الثلج) بمعدل 3 جلسات أسبوعيًا لمدة 4 أسابيع. المجموعة (ب) [مجموعة الدراسة]: تلقت الحجامة الجافة مع العلاج التقليدي(تمارين تقوية ساكنه، تمارين استطاله ،تمارين تصحيح القوام، وكمادات الثلج) بمعدل 3 جلسات أسبوعيًا لمدة 4 أسابيع.المجموعة (ج) [المجموعة الضابطة]: تلقت العلاج التقليدي فقط(تمارين تقوية ساكنه ، تمارين استطاله ، تمارين تصحيح القوام، وكمادات الثلج) بمعدل 3 جلسات أسبوعيًا لمدة 4 أسابيع. تم تقييم المرضى قبل وبعد العلاج باستخدام مقياس التناظر البصري (VAS) لقياس شدة الألم، وجهاز CROM لقياس مدى حركة الرقبة، ومقياس الخطأ في الإحساس الموضعي لقياس الإدراك الحركي، بالإضافة إلى مؤشر عجز الرقبة العربي (ANDI) لقياس الحالة الوظيفية.<br/>النتائج: أظهرت المجموعات الثلاث تحسنًا في جميع المتغيرات المدروسة. أظهرت المجموعة (أ) تحسنًا في شدة الألم مماثلًا للمجموعة (ب)، بينما كان تحسن مدى الحركة متساويًا في كل من المجموعتين (أ) و(ب). ومع ذلك، أظهرت المجموعة (أ) تفوقًا في تحسين الوظيفة والإحساس الموضعي.<br/>الاستنتاج: يعد التحريك اليدوى للأنسجة الرخوة باستخدام الاداه المساعده علاجًا فعالًا لتحسين الألم الميكانيكي المزمن في الرقبة.<br/>الأثر السريري لإعادة التأهيل: أسهمت التحريك اليدوى للأنسجة الرخوة باستخدام الاداه المساعده في تقليل شدة الألم وتحسين مدى الحركة والإحساس الموضعي والحالة الوظيفية لدى مرضى الألم الميكانيكي المزمن في الرقبة. لذلك يُوصى بإدراجها ضمن برامج إعادة التأهيل المخصصة لهؤلاء المرضى. |
