| Summary, etc. | Aim: To evaluate anterior temporalis and masseter muscle activity and patient satisfaction after using the Leaf Gauge (LG), Lucia Jig (LJ), and Kois Deprogrammer (KD) to locate centric relation (CR) in healthy adults.<br/>Methods: Thirty healthy adults were randomly allocated to three sequences for implementing tested methods, with a 24-hour wash-out period. Surface electromyography (Myowise) measured muscle activity at rest (R), maximum intercuspal position (MIP), and after locating CR. Patient satisfaction was assessed using a visual analogue scale (VAS) and a questionnaire. Friedman’s test followed by Dunn’s test compared muscle activity and VAS scores between methods (α=0.05).<br/>Results: Inter-method comparison showed no significant differences in anterior temporalis activity. Intra-method comparison showed that CR muscle activity was lower than MIP but higher than rest for all methods (P<0.001 for right, left, and bilateral means). For masseter muscle activity, KD showed significantly lower muscle activity compared to LG and LJ. LJ and KD activity at CR was comparable to rest and significantly lower than MIP. LJ was rated the most comfortable, while LG presented the least comfort (P=0.018). KD and LJ effectively reduced muscle activity when used to locate CR.<br/>Conclusion: LJ is simple and comfortable for healthy adults, while LG has limited versatility. Low muscle activity while locating CR is essential, as elevated muscle activity might compress the articular disc and alter the position. |

| Summary, etc. | تقصّت هذه التجربة الإكلينيكية العشوائية ذات التصميم المتقاطع ثلاثةَ أساليب لتحديد علاقة الإطباق المركزي (ع.م). وهي: المقياس الورقي، وأداة لوسيا، وجهاز كويس لإلغاء برمجة العضلات، وذلك لمعرفة تأثيرها على نشاط عضلتي الصدغي الأمامي والماهية (الماضغة)، بالإضافة إلى تقييم رضا المرضى. أكمل خمسةٌ وعشرون متطوع جميع الطرق الثلاث بترتيبات عشوائية على مدى ثلاثة أيام، مع فترة نقاهة مدتها 24 ساعة بين كل طريقة وأخرى. استخدمت تقنية تخطيط كهربية العضلات السطحي لتسجيل النشاط العضلي في وضع الراحة، ووضع الإطباق الأقصى، ومباشرة بعد تحديد ع.م. بكل طريقة.<br/>أظهرت الدراسة عدم وجود فروق ذات دلالة إحصائية بين الطرق الثلاث فيما يتعلق بنشاط عضلة الصدغي الأمامي عند قياسها في وضع ع.م.، مما يشير إلى أن اختلاف تصميم هذه الأجهزة لم يؤثر على المتطلبات العصبية العضلية لهذه العضلة. وعلى النقيض من ذلك، كان نشاط عضلة الماضغة في وضع ع.م. أقل بشكل ملحوظ عند استخدام جهاز كويس لإلغاء برمجة العضلات مقارنة بكلٍّ من المقياس الورقي وأداة لوسيا، ويُعزى ذلك إلى سهولة ثبات جهاز الإلغاء داخل الفم، مما يقلل الحاجة لانقباض العضلات لتثبيته.<br/>في جميع الطرق، سجّل وضع الإطباق الأقصى أعلى نشاط عضلي بشكل ثابت، بينما كان نشاط العضلات في وضع ع.م. أقل، وسُجّل أدنى نشاط في وضع الراحة. وعند مقارنة نشاط ع.م. بالراحة، أظهرت عضلة الصدغي الأمامي نشاطًا أعلى في ع.م.، ربما لأنها تشارك في إقحام لقمة الفك في موضعها أثناء تحديد العلاقة المركزية. أما أداة لوسيا وجهاز كويس لإلغاء برمجة العضلات فقد أظهرا نشاطًا لعضلة الماضغة في ع.م. مماثلًا تقريبًا لوضع الراحة، مما يعكس كفاءتهما في تقليل نشاط عضلات الرفع. بينما أظهر المقياس الورقي، بسبب افتقاره لوسيلة تثبيت داخل الفم، نشاطًا أعلى لعضلة الماضغة في ع.م. مقارنة بالراحة، ويرجّح أن ذلك يعود لاضطرار المرضى للحفاظ على الجهاز في مكانه باستخدام الجهد العضلي.<br/>أما فيما يتعلق برضا المرضى، فقد وُجدت فروق ذات دلالة إحصائية بين الطرق الثلاث. كانت أداة لوسيا الأكثر راحة، تليها جهاز كويس لإلغاء برمجة العضلات، بينما كان المقياس الورقي الأقل راحة. كما كانت درجات الرضا لأداة لوسيا وجهاز كويس أعلى بشكل ملحوظ من المقياس الورقي. ولم تُسجَّل فروق ذات دلالة بين الطرق الثلاث فيما يتعلق بسهولة اتباع التعليمات أو الإحساس بالألم.<br/>عالجت هذه الدراسة عددًا من القيود التي ظهرت في الأبحاث السابقة، لا سيما تلك المتعلقة بوضعية المريض وإجراءات إلغاء البرمجة العصبية العضلية وفترات النقاهة، مما جعل نتائجها أكثر قوة. وتدعم النتائج فعالية أداة لوسيا وجهاز كويس لإلغاء برمجة العضلات في تقليل النشاط العضلي أثناء تحديد ع.م.، مع إبراز محدودية المقياس الورقي. اشارت الدراسة إلى أن جهاز كويس وأداة لوسيا هما الخياران الأفضل لتحديد ع.م. لدى البالغين الأصحاء، مع تفوق أداة لوسيا في مستوى الراحة وسهولة الاستخدام. كما أوصى الباحثون بمزيد من الدراسات على متطوعين غير اصحاء، واقترحوا تقييم موضع لقمة الفك ثلاثي الأبعاد في الأبحاث المستقبلية لفهم التغيرات المرتبطة بـ ع.م. بصورة أدق. |
