| Summary, etc. | Background: Preoperative pregabalin is effective in reducing acute postoperative pain, prolonging nerve block duration, and sparing opioid use, while amantadine, an NMDA receptor antagonist, is widely used as an antiviral and in neurological disorders. This study aimed to evaluate the additive effect of amantadine to pregabalin in preventing post-thoracotomy pain syndrome (PTPS) and to assess postoperative quality of life and functional activity using the Barthel Activities of Daily Living scale (ADL).<br/>Methods: This randomized controlled trial included 60 cancer patients undergoing thoracotomy or lung resection under general anesthesia. Patients were allocated into two equal groups: Group A: received Amantadine 100 mg with Pregabalin 75 mg two hours preoperatively, followed by both drugs twice daily for 5 days postoperatively. Group P: received Pregabalin 75 mg with a placebo capsule resembling Amantadine two hours preoperatively, then continued twice daily for 5 days postoperatively.<br/>Results: Need of fentanyl consumption, total fentanyl consumption, need of morphine consumption and total morphine consumption in the first 48 hours were insignificantly different between both groups. Sedation was insignificantly different between both groups. Grading system for neuropathic pain was significantly lower at 4 weeks, 8 weeks and 12 weeks in group A than group P. Quality of life according to Flanagan scale, ADL and patient global impression of change were insignificantly different at baseline, 2 weeks, 4 weeks, 8 weeks and 12 weeks between both groups. Postoperative nausea and vomiting and complications were insignificantly different between both groups.<br/>Conclusion: The combination of amantadine to pregabalin demonstrated a significant reduction in pain scores and neuropathic pain grading at later follow-up, suggesting a potential long-term benefit in mitigating post-thoracotomy pain syndrome. |

| Summary, etc. | المقدمة: يُعد البريجابالين قبل الجراحة فعّالًا في تقليل الألم الحاد بعد العمليات الجراحية، وإطالة مدة تأثير الحصار العصبي، وتقليل الحاجة لاستخدام المواد الأفيونية، بينما يُستخدم الأمانتادين، وهو مضاد لمستقبلات إن-ميثيل-دي-أسبارتات، على نطاق واسع كمضاد فيروسات وفي الاضطرابات العصبية. هدفت هذه الدراسة إلى تقييم التأثير الإضافي للأمانتادين عند دمجه مع البريجابالين في الوقاية من متلازمة الألم بعد شق الصدر، وكذلك تقييم جودة الحياة بعد الجراحة والنشاط الوظيفي باستخدام مقياس بارتل لأنشطة الحياة اليومية.<br/>الطريقة: شملت هذه التجربة العشوائية المحكمة 60 مريضًا مصابًا بالسرطان خضعوا لجراحة شق الصدر أو استئصال جزء من الرئة تحت التخدير العام. تم تقسيم المرضى إلى مجموعتين متساويتين: المجموعة (أ): تلقي المرضي 100 مللي جرام من الأمانتادين مع 75 مللي جرام من البريجابالين قبل الجراحة بساعتين، تلاها تناول كلا الدوائين مرتين يوميًا لمدة 5 أيام بعد الجراحة. المجموعة (ب): تلقي المرضي 75 مللي جرام من البريجابالين مع كبسولة دواء وهمي تشبه الأمانتادين قبل الجراحة بساعتين، ثم استمرت في تناول الدواء مرتين يوميًا لمدة 5 أيام بعد الجراحة.<br/>النتائج: كانت الحاجة لاستخدام الفنتانيل، وإجمالي استهلاك الفنتانيل، والحاجة لاستخدام المورفين، وإجمالي استهلاك المورفين خلال أول 48 ساعة غير مختلفة بشكل ذي دلالة إحصائية بين المجموعتين. كما لم يكن هناك فرق ذو دلالة إحصائية في مستوى التهدئة بين المجموعتين. كان تقييم شدة الألم العصبي أقل بشكل ملحوظ في الأسابيع الرابعة والثامنة والثانية عشرة في المجموعة (أ) مقارنة بالمجموعة (ب(. أما جودة الحياة حسب مقياس فلاناغان، ومقياس أنشطة الحياة اليومية، والانطباع العام للمريض عن التغيير، فكانت غير مختلفة بشكل ذي دلالة إحصائية عند خط الأساس، وبعد أسبوعين، وأربعة أسابيع، وثمانية أسابيع، واثني عشر أسبوعًا بين المجموعتين. كذلك، لم يكن هناك فرق ذو دلالة إحصائية في معدل حدوث الغثيان والقيء بعد الجراحة والمضاعفات بين المجموعتين.<br/>الاستنتاج: ظهر دمج الأمانتادين مع البريجابالين انخفاضًا ملحوظًا في درجات الألم وتقييم الألم العصبي في فترات المتابعة المتأخرة، مما يشير إلى فائدة محتملة طويلة المدى في التخفيف من متلازمة الألم بعد شق الصدر. |
