| Summary, etc. | Background: <br/><br/>Weaning from mechanical ventilation is an everyday practice for<br/>intensivists; thus, this decision should be carefully anticipated. We aimed<br/>to evaluate the feasibility and reproducibility of scalenus anterior<br/>thickening fraction as a potential detector of weaning outcomes. <br/>Method: <br/>We conducted two separate sub-studies; sub-study A aimed to<br/>evaluate the feasibility and reproducibility of scalene muscle<br/>examination, and sub-study B evaluated the diagnostic accuracy of<br/>scalenus anterior thickening to predict weaning outcome. <br/>Results: <br/>Scalenus anterior thickening has good intra- and inter-observer<br/>reliability. It also showed good accuracy in predicting failed SBT and<br/>reintubation. <br/>Conclusion: <br/>Scalenus anterior examination is a feasible tool with good<br/>interobserver reliability. Scalenus anterior thickening fraction could<br/>accurately predict weaning outcomes. |

| Summary, etc. | فشل نزع أنبوب التنفس (الفشل في الفطام من جهاز التنفس الصناعي) يعدّ حالة حرجة في ممارسة الرعاية الجراحية. .نفترض أن زيادة سُمك العضلة الاخميعة الامامية قد تكون امؤشر حساسًا للتنبؤ بنجاح عملية الفطام من جهاز التنفس الصناعي. <br/>نهدف الي تقی#م فعالية زيادة سُمك العضلة الاخميعة الامامية في التنبؤ بنجاح الفطام من جهاز التنفس الصناعي لدى المرضى المصابین :بامراض حرجة.. <br/>المستقبلية الملاحظة شملت مرضى بالغين خضعوا للتهوية الميكانيكية لمدة 24 ساعة أو أكثر.هذه الد ارسة تم فحص 75 مريضًا جاهزين للفطام للتحقق من مطابقتهم لمعاییر الاشتمال، وتم استبعاد 9 حالات، لیُدرج في التحلیل النهائي 66 مر2ضًا .من بین هؤلاء، نجحت عملية الفطام في 48 مر2ضًا، بینما فشلت في 18 مر2ضًا، واحتاج 7 مرضى إلى إعادة التنبیب. <br/>تم حساب القيم المتوسطة لكلٍّ من الإ ازحة ونسبة زيادة سُمك العضلة الاخميعة الامامية كمعدل بین الجانبین الأيمن والأيسر. <br/>اظهرتحلیل منحني الخصائص التشغیلية للمستقبل لنسبة زيادة سمك العضلة الاخميعة الامامية دقة جیدة في التنبؤ :فشل اخت:ار التنفس التلقائي حیث بلغت المساحة تحت المنحني (فاصل الثقة 95%) 0.92 (0.82–1) للجانب الأيمن و0.94 (0.84–0.98) للجانب الأيسر :الإضافة إلى ذلك، =انت نس:ة ز2ادة سُمك العضلة الاخميعة الامامية قادره علي التنبؤ :الحاجه الي اعادة التتییب بدقه (1.00) وفي الجانب الأيمن0.99( في الجانب الأيسر.عاليه حیث بلغت المساحة تحت المنحنى قيمة سمك العضله الاخميعة الامامية اقل من 10% يمكن ان تستبعد فشل الفطام بنس:ة قيمه تنبؤية سلبيه تبلغ 100%. <br/>و د كانت القيم الحدية بنسبة سمك العضله الاخميعة الامامية اليمنى في التنبؤ بفشل محاولة التنفس التلقائي و اعادة التتیب 21% و 12% على التوالي في حین =انت القيمه الحديه لنسبة سمك العضله الاخمعيه. |
