Summary, etc.	Aim: <br/>Oral papillary squamous cell carcinoma (OPSCC) is a rare subtype of squamous cell carcinoma with debatable prognosis and metastatic potential. To better understand this neoplasm, it is essential to investigate its biological behavior, growth patterns, and tumor microenvironment. Our study aims to compare the expression of toll-like receptor 4 in papillary and various grades of conventional squamous cell carcinoma.<br/>Materials & Methods:<br/>This comparative cross-sectional study included formalin-fixed paraffin-embedded samples of different grades of conventional squamous cell carcinoma, OPSCC, and normal epithelial tissue as a control group. TLR4 mRNA levels were evaluated using RT-qPCR, while protein expression was assessed by immunohistochemical staining. Statistical analysis was performed to compare expression between the groups.<br/>Results:<br/>TLR4 exhibited cytoplasmic and variable nuclear expression in both OSCC and OPSCC, with the highest levels found in poorly differentiated SCC. RT-qPCR confirmed significantly elevated TLR4 mRNA in all cancer groups compared to controls. No statistically significant differences in TLR4 expression were observed between OPSCC and well-differentiated or moderately differentiated squamous cell carcinoma in either immunohistochemical or PCR analysis.<br/>Conclusion:<br/>The results indicate that while OPSCC may display unique biological characteristics compared to conventional OSCC, its behavior appears to resemble that of OSCC with similar histological grading, regardless of connective tissue invasion.

Summary, etc.	تهدف هذه الدراسه الى مقارنة مستويات التعبير للشبيه بالتول فى سرطان الخلية الحرشفيه الحليميه الفموى مقارنة بسرطان الخلية الحرشفية. يُعد سرطان الخلايا الحرشفية من الأورام الخبيثة الشائعة التي تتضمن عدة أنواع نسيجية، ويُظهر النوع الحليمي منها خصائص مورفولوجية وسريرية مميزة.<br/>تم جمع 42 عينة بارافين ارشيفية ، شملت 12 عينة من الأنسجة الطبيعية، و12 حالة من سرطان الخلايا الحرشفية الحليمي، و18 حالة من سرطان الخلايا الحرشفية التقليدي. تم تصنيف هذه العينات وفقًا لتصنيف منظمة الصحة العالميه لاورام الرأس و العنق لعام 2022 الى 6 حالات من النوع الجيد التمايز و 6 حالات من نوع متوسط التمايز و 6 حالات من ضعيف التمايز.<br/>تم تقييم التعبير عن الاجسام المضادة للشبيه بالتول باستخدام الكيمياء الهيستولوجيه المناعية ثم تم قياس النسبة المئوية للمساحة المعبرة عن شبيه التول ببرنامج لايكا كوين 500 . كما تم استخدام تفاعل البوليمراز المتسلسل للكشف عن انماط التعبير الجينى.<br/>اظهرت الدراسه الحاليه وجود شبيه التول فى جميع حالات سرطان الخلايا الحرشيفيه التقليدى و الحميمى بينما ظهر 16% من مجموعه التحكم بالكيمياء الهيستولوجيه المناعية . حالات سرطان الخلايا الحرشفية ضعيف التمايز سجلت أعلى متوسط نسبة مئوية للمساحة. في حين سجلت مجموعه التحكم أقل قيمة.<br/>كما أظهر التحليل الإحصائي عدم وجود فروق ذات دلالة إحصائية بين حالات السرطان التقليدي جيد التمايز وسرطان الخلايا الحرشفية الحليمي، وكذلك بين النوع متوسط التمايز وسرطان الخلايا الحرشفية الحليمي. وأكدت نتائج تفاعل البوليمراز المتسلسل وجود تعبير جيني لشبيه التول في حالات السرطان ضعيف التمايز، دون وجود فرق ذو دلالة احصائية بين النوع جيد التمايز وسرطان الخلايا الحرشفية الحليمي.<br/>ولقد استنتجت الدراسه ان سرطان الخلايا الحرشفية الحليمي يُظهر سلوكًا حيويًا مشابهًا لسرطان الخلايا الحرشفية التقليدي عند نفس الدرجات النسيجية، ويمكن استخدام شبيه التول لتقييم درجة تمايز الورم والمساعدة في وضع استراتيجيات علاجية.
