| Summary, etc. | Introduction: Dementia poses a significant public health challenge, with high rates of undiagnosed cases, particularly in developing countries. Incorporating cognitive assessment into time-limited primary care visits is difficult as it consumes the limited physician’s time.<br/>Aim: This study evaluates the diagnostic accuracy of the Self-Administered Gerocognitive Examination (SAGE) in detecting mild cognitive impairment (MCI) and early dementia (ED) among elderly, using the Montreal Cognitive Assessment-Basic (MoCA-B) as the reference standard.<br/>Methods: A total of 209 elderly individuals (aged 60 and above) attending Kasralainy family medicine clinic were recruited. Participants completed the self-administered Arabic version of SAGE, followed by the clinician-administered Arabic version of MoCA-B. Socio-economic and clinical data were also collected. <br/>Results: SAGE demonstrated sensitivity 91.30%, specificity 60.31%, positive predictive value 44.68% and negative predictive value 95.18% in predicting mild neurocognitive disorder (MCI). It exhibited sensitivity 100%, specificity 63.28%, positive predictive value 32.99%, and negative predictive value 100% in predicting major neurocognitive disorder (dementia). <br/>There is a strong positive correlation between the total scores of SAGE and MoCA-B (r =0.744, p value <o.oo1). The orientation (r=0.991) and construction domains (r=0.888) showed strong correlations while other domains showed weaker correlations. <br/>Based on MoCA-B, the frequency of NCD in our study was 37.3% (MCI 22.0%, dementia 15.3%). Older age was the strongest factor linked to lower scores on both the SAGE and MoCA-B assessments, whereas higher education and having a professional job were closely related to better cognitive performance. Additionally, vascular conditions such as hypertension, diabetes, and cerebrovascular accidents were associated with notably reduced scores on both tests.<br/>Conclusion: SAGE is a highly sensitive self-administered tool for screening for cognitive impairment and it is best recommended as a first-step screen in a stepwise diagnostic pathway. Patients who yield positive results on SAGE should proceed to confirmatory testing. |
