Summary, etc. Hemodialysis patients need aid in various physiological and psychosocial issues they could face. Also lifestyle changes such as; role change, trouble to save a job, prudent load of hemodialysis medication, challenges to buy all those medications and being constantly chronically sick could make sense of lack of controlling the illness. Self care program empowers patients to move toward their self-care abilities for making proficient decisions, and assuming greater personal responsibility for their health. So the aim of the study was evaluate the impact of a designed self-care program on selected outcomes among patients undergoing hemodialysis. A convenience sample of 60 patients with chronic kidney disease stage five (CKD5) undergoing hemodialysis (HD) was assigned randomly into two groups; study and control group, 30 patients for each group was selected during one year in hemodialysis unit at New Teaching University Hospital. Quasi-experimental design (pre-post test) was chosen to conduct this study. Four tools utilized for the study; (1) semi-structured scheduled interviews include sociodemographic data sheet, patients' medical history and physiologic data measures. (2) A structured interview questionnaire (3) Nottingham extended activities of daily living (ADL) scale (4) structured observation checklists. The results of the study showed that there were statistical significant differences in the total mean scores of knowledge, Nottingham (ADL) scale and total practice of the patients in the study group compared to the control group and compared to their own baseline in the post and follow up periods. There were statistical significant differences regarding hematocrit, hemoglobin and blood urea nitrogen levels between study and control group in post and follow up periods. The study concluded that the designed self care program was effective for improving hemodialysis patients' knowledge, practice and independency level. The study recommended need for adopting self care program as part in the protocol of care for hemodialysis patients and train nurses regarding the program
