Summary, etc. Acne keloidalis nuchae is an idiopathic, scarring folliculitis characterized by the formation of papules and pustules that may ultimately develop into tumor-like masses distributed on the nape of the neck and occipital region of the scalp. While the pathogenesis remains uncertain, precipitating factors include localized trauma, chronic irritation and seborrhea may play a role in affecting the hair follicle and development of the disease. The treatment of acne keloidalis nuchae is challenging and depends on the clinical stage of the disease; however, a multifaceted approaches involving combination therapies (medical, surgical or laser) have proven to be effective. Unfortunately acne keloidalis may persists despite a variety of treatments.The aim of this study was to assess the effect of 755 nm alexandrite laser in the treatment of different lesions of AKN. Twenty male patients diagnosed as AKN received six successive sessions of laser treatment at four weeks interval and were followed up three months after the final treatment session.The effect of treatment was evaluated clinically through papule and pustule count, keloidal plaque size and pliability assessment. Also patient self- assessment included evaluation of pain, pruritus and degree of satisfaction was evaluated. It was found that there is improvement in the lesion regarding the previous parameters with improvement of subjective symptoms such as pruritus and pain. Reduction of hair density in treated area is the only side effect. There was a highly statistically significant difference between before and after treatment regarding most of the parameters of the lesion
