Summary, etc. Background: High levels of anxiety are associated with worse outcomes in coronary artery disease patients. However little is known about anxiety levels in patients undergoing coronary procedures. Objective : The aim of this study was to assess the anxiety level of patients undergoing elective coronary angiography/angioplasty for the first time and observe it's effect on the procedural outcomes. Subjects and Methods: This sample study included 102 patients presenting to the Catheterization lab of Cardiovascular Department in Cairo University hospital between the time period from 15th June 2019 and 2nd December 2019. All patients were assessed by Hamilton anxiety (HAM-A) and general healthy questionnaire 12 (GHQ12) screening tools. Results: Mean age of the study population was 55 ±7.22 years. The prevalence of mild and moderate-severe levels of anxiety was 55.9% and 44.1% respectively. Moderate/severe anxiety was more common in females (0.003). Moderate/severe anxiety was significantly associated with hypertension, diabetes mellitus and unemployed patients P= 0.001, 0.005, 0.011 respectively. The highest anxiety levels was not associated with procedural success, duration of procedure, change in vascular access and associated complications. The highest anxiety levels were significantly correlated with intra-operative SBP, DBP and pulse P= 0.006 , 0.022, 0.004 respectively. Female sex, hypertension and GHQ12 were an independent predictors for moderate/severe anxiety P= 0.031, 0.003 and 0.003 respectively. Conclusion: hemodynamic derangement (SPB, DBP and pulse) are associated with high anxiety levels. Detailed pre procedural assessment together with psychological and pharmacological plans could decrease anxiety level and improve the outcomes in these patients
