Thesis (Ph.D)-Cairo University, 2023.
Bibliography: pages 103-128.
Introduction: Premature ejaculation (PE) is the most prevalent sexual dysfunction among males, with a prevalence of 5% to 40% of sexually active men.
Aim of the study: The aim of the study is evaluating the use of TPTNS in treatment of PE as regards efficacy and safety using the Intravaginal Ejaculatory Latency Time (IELT) and Arabic index of premature ejaculation (AIPE)
Methods: This study was carried out on (50) male patients, complaining of premature ejaculation either primary or secondary with IELT less than 2 minutes recorded by digital hand watch, attending the outpatient clinic of Andrology department, Kasr Al Ainy Hospital, Cairo University. They were divided into:
Group (A): patients with PE that will undergo 10 sessions of transcutaneous posterior tibial nerve stimulation (TPTNS) on alternating days over a period of three weeks.
Group (B): patients with PE that will undergo 10 sessions of sham Transcutaneous electrical nerve simulation (TENS) on alternating days over a period of three weeks.
All patients were subjected to full history taking including (international index of erectile function-5 (IIEF-5), and AIPE questionnaire assessments), clinical examination. The patient was instructed to use a digital hand watch during the time of intercourse and to calculate the time difference between the start of intromission and time of ejaculation.
Results: Current study showed significant improvement in IELT after receiving TPTNS with median IELT fold increase 3.095. Sixteen patients experienced improvement in their AIPE score in TPTNS group (72.72%) compared with four in the sham TENS group (18.18%), P<0.001.
Conclusion TPTNS is safe non-invasive procedure and has a promising potential to be one of the reliable line of treatment for PE as it significantly improved both IELT and AIPE score.
