Purpose: to compare the superficial and deep vascular densities and the foveal avascular zone parameters using OCTA with and without ILM peeling in PDR patients undergoing PPV. Also to compare both groups regarding postoperative BCVA, incidence of ERM formation and occurrence of macular edema.
Study design: A prospective randomized controlled clinical trial.
Patients and Methods: thirty-six PDR patients having vitreous hemorrhage or tractional retinal detachment threatening the macula were randomly divided into 2 equal groups. Group A patients underwent PPV with ILM peeling whereas group B patients did not undergo ILM peeling. OCTA was done 3 and 6 months postoperatively
Results: the study demonstrated a significantly higher superficial parafoveal total VD, superficial parafoveal inferior hemi VD, superficial parafoveal temporal VD and superficial parafoveal inferior VD in the ILM non-peeling group compared to the ILM peeling group (p-value=.003, p-value=.001, p-value<.001 and p-value=.005, respectively), however, no statistically significant difference in the deep vascular densities or the FAZ parameters between both groups after 6 months. Regarding the visual outcome, there was no statistically significant difference in the BCVA at 3 and 6 months in both groups, however, patients in the non-peeling group had a statistically significant higher incidence of ERM formation 61.1% vs 5.6% in the ILM peeling group.
Conclusion: The current study concluded that ILM peeling in PDR patients undergoing PPV may alter the superficial parafoveal vascular densities but with a much lower risk of ERM formation compared to non-peeling at 6 months postoperatively. Despite these differences, the final visual acuity was similar in both groups

إزالة الجسم الزجاجي هي العملية الجراحية الاساسية لعلاج مضاعفات اعتلال الشبكية السكري التكاثري بعد فشل الليزر الكلى للشبكية وحقن مضادات عامل نمو بطانة الأوعية الدموية.مع العلم ان تقشير الغشاء المحدد الداخلي هي خطوة جراحية تستخدم في العديد من أمراض الشبكية للقضاء على جميع قوى الشد على سطح الشبكية ولإزالة المسطح الذي تستخدمه الخلايا الليفية العضلية للتكاثر على سطح الشبكية وبالتالي منع تكوين تليفات على سطح الشبكية.
هذه الدراسة عبارة عن تجربة سريرية مستقبلية وعشوائية ومضبوطة في مركز واحد.هذه الدراسة تتضمن 36 عينًا لـ 36 مريضًا من مرضى اعتلال الشبكية السكري التكاثري الذين استوفوا معايير الاشتمال من عيادة الشبكية التخصصية بقصر العيني، مستشفيات جامعة القاهرة. تم اختيار المشاركين بشكل عشوائي للخضوع إما لإزالة الجسم الزجاجي مع تقشيرالغشاء المحدد الداخلي أوإزالة الجسم الزجاجي بدون تقشيرالغشاء المحدد الداخلي وتمت متابعة المرضى لمدة 6 أشهر. تم استخدام الماسح الضوئي المقطعي المترابط المصور للأوعية لمقارنة تأثير الجراحة على كثافات الأوعية الدموية السطحية والعميقة ومنطقة النقرة اللاوعائية 3 و 6 أشهر بعد الاجراء الجراحي.
كشفت الدراسة الحالية وجود كثافة أعلى في الأوعية الدموية السطحية حول النقرة حين لا يتم تقشيرالغشاء المحدد الداخلي لكن لا يوجد فرق ذو دلالة إحصائية في كثافات الأوعية الدموية العميقة أو منطقة النقرة اللاوعائية بين كلا المجموعتين بعد 6 أشهر.
و فيما يتعلق بحدة الابصار، أظهرت نتائج الدراسة الحالية عدم وجود فروق ذات دلالة إحصائية في حدة الابصار بعد 3 و 6 أشهر من اجراء جراحة إزالة الجسم الزجاجي في كلتا المجموعتين، ومع ذلك، كان لمرضى إزالة الجسم الزجاجي بدون تقشيرالغشاء المحدد الداخلي نسبة أعلى ذات دلالة إحصائية لتكوين غشاء تليفي على سطح الشبكية
