Background: Cervical corpectomy is most commonly required for decompression of the anterior spine in the treatment of degenerative spondylosis, trauma, primary or metastatic tumors, osteomyelitis, kyphosis or deformity, and ossification of the posterior longitudinal ligament (OPLL). Following corpectomy, reconstruction of the anterior column is required, and various approaches have been used previously. The use of expandable cages has been described in the literature for over 10 years.
Methodology: This prospective study conducted on 15 patients with cervical vertebral body pathology fit for surgical corpectomy as verified clinically and radiologically. Expandable cages were used in all patients with anterior plating. Clinical follow-up with clinical examination, assessment of the visual analogue scale (VAS) and the Nurick score at 3, 6 months after surgery. Radiological follow-up with X-ray immediately post-op and 3 months after, CT scan after 6 months to assess fusion, subsidence, and the Cobb C2–C7 angle. All patients operated upon in the Neurosurgery Department, Cairo University Hospitals in the period from November 2022 to January 2024.
Results: Clinical outcome of all 15 patients reported to be improved by VAS and Nurick score. There was improvement of cobb’s angle and lordotic curve, 80% achieved fusion, 33% of patients noted to have subsidence on follow up.
Conclusion: Expandable cages appear to provide good structural support for various conditions in the cervical spine. Single-level surgery showed more favorable clinical outcomes. The good clinical results with neurological improvement and resolution of pain, the high rate of fusion, and low rates of subsidence, complications make the expandable cages an attractive tool for replacing the vertebral body.

تُستخدم الأقفاص القابلة للتمدد لإعادة بناء العمود الفقري الأمامي بعد استئصال الجسم الفقاري. وتشمل المؤشرات تضيق القناة الشوكية مع الضغط على الحبل الشوكي، والكسر، والتهاب القرص الفقري، والنقائل.
يتم إجراء استئصال الجسم الفقاري للعمود الفقري العنقي من خلال النهج الأمامي مع الحاجة إلى قفص أو استعواض عظمي لإعادة البناء. يمكن تعديل الأقفاص القابلة للتمدد لتناسب حجم استئصال الجسم الفقري، وهي توفر دعمًا أماميًا قويًا، مما يوفر ثباتًا أوليًا ممتازًا مع انخفاض خطر تلف اللوحة النهائية، وتقليل التلاعب بالجهاز أثناء الجراحة، والتحكم بشكل أكبر في التقوس الفقري.
أجريت دراستنا على 15 مريضًا خضعوا لاستئصال الجسم الفقاري العنقي ، مع متابعة سريرية باستخدام مقياس VAS ودرجات Nurick التي تبين أنها كانت ذات نتائج إيجابية باستخدام الأقفاص القابلة للتوسيع. تم الإبلاغ عن انخفاض الألم وتحسن العجز العصبي.
المتابعة بالاشعه باستخدام الأشعة السينية والمسح المقطعي المحوسب مباشرة بعد العملية الجراحية، وبعد 3 و6 أشهر لفحص الانحناء العنقي والاندماج والهبوط، تبين أنها ذات قيمة جيدة لاستخدامها في مثل هذا الإجراء.
زادت زاوية كوب في المدى بعد الجراحة مقارنة بحالة ما قبل الجراحة مع التصحيح الجيد للانحناء العنقي.
في دراستنا، أثبتت صور الأشعة السينية المتابعة لـ 12 مريضًا (80%) تحقيق الاندماج عن طريق تكوين عوارض عظمية بين القفص المتمدد والأجسام الفقارية المجاورة، بينما فشل 3 مرضى في الاندماج وتم ملاحظة التهاب المفاصل الكاذب دون مزيد من التدهور في النتائج السريرية.
