Summary: Background: Low inflammatory and oxidative damage may be implicated in the pathophysiology of irritable bowel syndrome (IBS); hence, they may influence the onset and extent of IBS symptoms. Traditional therapeutic strategies focusing primarily on symptom alleviation are no longer effective; thus, several attempts are oriented toward treating etiology. Purpose: The study aimed to investigate the effect of Pilates exercises on selected inflammatory and oxidative stress biomarkers in patients with irritable bowel syndrome. Subjects and methods: Sixty women with constipation-predominant irritable bowel syndrome (IBS-C). Their ages ranged from 20 to 45 years and body mass index ranged from 25 34.9kg/m2 participated in the study. Patients were selected from the outpatient clinic of Tanta University Hospital, and the study was conducted at an outpatient physiotherapy clinic. The patients were randomly assigned to two equal groups: Group A (study group), which included 30 women with irritable bowel syndrome. They participated in 8 weeks of the Pilates exercise program (2 sessions per week) in addition to traditional dietary advice, and Group B (control group), which included 30 women with irritable bowel syndrome. They adhered to traditional dietary advice only for 8 weeks. Results: the study group showed more significant improvement than the control group in irritable bowel syndrome symptom severity score(↓66.75% versus ↓38.28%, Cohen d=0.73), number of complete spontaneous bowel movements ( ↑97.17 % versus ↑67.69%, Cohen d=0.50), fatigue (↓55.52% versus ↓28.83%, Cohen d=0.74), anxiety (↓53.45% versus ↓27.64%, Cohen d=0.56), and depression (↓59.1% versus ↓26.33%, Cohen d=0.64) while there were non-significant differences between groups in malondialdehyde, Tumor necrosis factor -α, and Bodyweight (p > 0.05). II Malondialdehyde significantly increased only in group A compared to baseline, and body weight reduced significantly only in group A compared to baseline (P < 0.05). Also, the control group showed significant improvements in all outcomes, except for malondialdehyde and body weight compared to baseline (P < 0.05). Conclusion: Combining Pilates exercises with traditional dietary advice may significantly improve the severity and frequency of irritable bowel syndrome symptoms, and improve the number of complete spontaneous bowel movements, and relieve fatigue, anxiety, and depression than dietary advice alone in women with constipation-predominant IBS, following traditional dietary advice alone may greatly enhance these parameters. However, adding Pilates exercises to traditional dietary advice offers no advantages over traditional dietary intervention in terms of oxidative stress and inflammatory indicatorsSummary: شارك في الدراسة ستون مريضة مصابة بمتلازمة القولون العصبي (الإمساك السائد)، وتراوحت أعمارهن بين 20 إلى 45 سنة ومؤشر كتلة الجسم يتراوح بين 25-34.9 كجم/م2. تم اختيار المرضى من العيادة الخارجية لمستشفى طنطا الجامعي.وأجريت الدراسة في عيادة خارجية للعلاج الطبيعي تم تقسيم المرضى عشوائياً إلى مجموعتين متساويتين: المجموعة أ (مجموعة الدراسة) والتي احتوت على 30 مريضة ممن يعانون من متلازمة القولون العصبي. وقد شاركوا في برنامج تمارين البيلاتس لمدة 8 أسابيع (جلستين أسبوعيًا) بالإضافة إلى النصائح الغذائية التقليدية والمجموعة ب (المجموعة الضابطة) التي احتوت على30 مريضًة . الجمع بين البيلاتس والنظام الغذائى التقليدى قد يحسن بشكل كبير من شدة أعراض القولون العصبي وتكرارها، وعدد مرات الافراغ الكامل والتلقائى للأمعاء أسبوعيا والتعب والقلق والاكتئاب أكثر من النصائح الغذائية وحدها لدى النساء المصابات بمتلازمة القولون العصبي السائد بالإمساك، ولكن الجمع بينهم لم تحدث أى فرق أضافى بالنسبة للإجهاد التأكسدي ومؤشرات الالتهابات علاوة على النصائح الغذائية وحدها.
