The aim of this study is to detect risk of atherosclerosis in Egyptian children with FMF, compared to healthy control and assess the value of atherogenic index as a predictor of atherosclerosis in FMF patients. This cross sectional study included 50 Egyptian children having FMF and in attack-free period, together with 40 healthy children as controls. Our results showed that mean cholesterol and low density lipoprotein (LDL) values were higher in FMF cases than controls with statistical significance difference. Mean high density lipoprotein (HDL) and triglyceride (TG) values were slightly higher in cases than controls, but with no significance difference. There was no significance difference between cases and controls regarding atherogenic index. Cholesterol, LDL and atherogenic index had significant correlations with ESR. This study concluded that risk of atherosclerosis is higher in Egyptian children with FMF than their matched controls and that subclinical inflammation might play a role to increase this risk
