Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of General Surgery Summary: Control of facial scarring is one of the most difficult challenges in surgical practice, and represents a difficult therapeutic problem facing plastic surgeons to achieve good results. To date, no gold standard exists for the treatment of scar tissue. Autologous fat grafting has been introduced as a promising treatment option for scar tissue related symptoms. However, the scientific evidence for its effectiveness remains unclear. This study was conducted on 40 patients with obvious facial scars. Patients{u2019} age ranged from 18 to 60 years. Patients were selected randomly to be treated with fat grafting. The abdomen was the most commonly chosen donor site. Fat was processed with centrifuge in 20 (group A) and unprocessed in the other 20 cases (group B). Fat grafting proved to have a significant role in scar remodeling. This was measured clinically by the vancouver scar scale. Regarding patient satisfaction with cosmetic appearance, 25 cases were evaluated as excellent, 10 in group A and 15 in group B 8 cases were evaluated as good, 3 cases in group A and 5 in group B and 7 cases were evaluated as fair group A
