Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Oral and Dental Medicine - Department of Oral and Maxillofacial Surgery Summary: This study aimed at the evaluation of the marginal bone loss associated with ridge augmentation through interpositional osteotomy (sandwich technique) in the posterior mandible with simultaneous implant placement to maintain the created gap and to stabilize the osteomatised segment. Six patients (one male &five females) with edentulous posterior mandible (free end saddle) were treated in this study. Radiographic assessment was performed preoperatively, one week postoperatively and six months post operatively .the assessment included the measurements of the linear changes in the marginal bone surrounding the placed implants, these measurements were performed on cross sectional and longitudinal cuts of cone beam computed tomography (CBCT) using special software. The mean bone resorption on the mesial side was 1.7mm representing 15.4% from the baseline, while on the distal side it was 1.2mm representing 17.6% from the baseline, on the buccal side it was 3.1mm representing 31.8% from the baseline and lastly on the lingual side the mean was 2.1mm representing 18.8% from the base line. From the results of the present study we found that interpositional osteotomy with immediate implant placement has proved to be a reliable technique in augmentation of the height of the bone in the posterior mandible but also with recommendation of using a bone substitute or autogenous bone graft in the created gap to compensate for the resorption in the buccal aspect
