Objective: This study was done to compare duodeno-endoscopic and histological findings in typical and atypical celiac disease in children and to describe different presentation of celiac disease. Patients and methods: Our study is a cross sectional analytic study carried out on 199 cases of celiac disease in outpatient Gastroenterology clinic at Abu El-Reesh children hospital, Cairo University from JUN 2019. Patients was divided into typical and atypical group according to presenting symptoms. Upper gastrointestinal endoscopy and duodenal biopsy were performed for patients who have symptoms suggestive of celiac disease. The severity of intestinal damage was graded by the pathologist as per the Marsh-Oberhuber classification. Results: Age was significantly higher in the atypical group (p value< 0.001). Chronic diarrhea was the main intestinal presentation in typical group. Anemia was the most common extraintestinal symptom in both typical and atypical group. Marsh stage 3b and 3c was significantly higher in seropositive patients with p-value 0.007. There were significant correlation between histological grade of biopsied duodenal mucosa and clinical presentation (p< 0.001)
