The mere term of adrenal incidentaloma (AI) by definition is an adrenal mass unexpectedly detected through an imaging procedure performed for reasons unrelated to adrenal dysfunction or suspected dysfunction. The preferred approach to their management in terms of diagnosis, follow-up, and treatment remain controversial. Despite their frequent appearance, the challenge remains to recognize and treat the small percentage of AI that do pose a significant risk, either because of their hormonal activity or because of their malignant histology (Aron et al, 2012). Although adrenal lesions are a common imaging finding, the vast majority of adrenal lesions are benign adenomas, which contain intracytoplasmic (microscopic) fat (Adam et al, 2017). Other pathologies are present (including pheochromocytoma, adrenocortical carcinoma [ACC] , and metastatic disease)
