Background: Secondary bone grafting has been used for alveolar cleft closure. However, grafting material to be used is still controversy. Objective:The aim of the study is to evaluate the effectiveness and complications of mineralized plasmatic matrix prepared from allogenic bone graft used in secondary alveolar cleft grafting. Patients and Methods: This prospective randomized study included 66 patients with alveolar clefts.They were classified into two equal groups; group (1) included 33 patients in whom grafting with anterior iliac crest bone was used (Control group) and group (2) included 33 patients in whom grafting with MPM prepared from allogenic bone graft (study group). Results: The outcome of the studied patients 3 months postoperatively; 31 patients (93.93%) and 32 patients (96.97%) had complete cleft closure in the control group (1) and MPM group (2), respectively, while partial cleft closure was observed in only 2 cases (6.06%) and 1 patient (3.03%) in groups (1) and (2), respectively. No cases had operation failure in both groups. Regarding the bone graft volume after 6 months follow up period, they all showed statistically non-significant difference (p >0.05) in comparison between both groups. The extent of postoperative pain at the bone graft donor site evaluated by VAS. In patients underwent iliac crest bone graft harvesting, all levels of VAS score had a statistically significant difference (P <0.05) in comparison between pain at first, third and six months postoperatively. Levels 1, 9 & 10 VAS score had no data. The mean VAS score was 3 ± 7.403, 3 ± 5.916, 3 ± 6.309 in the first, third and six months postoperatively with statistically non-significant difference (P = 0.142)
