Short - scar cervicofacial rhytidectomy has been used with great success and a high level of patient satisfactionThe operation involves limited incisions in the sideburn and preauricular areas , extending just around the fold of the ear lobuleThe operation is safe , speedy , less traumatic , and well tolerated among patients at high risk , such as smokersThe risks of complications are also reduced with this operationAnother technique is a MACS lift which can be done through an inverted L - shaped preauricular incision with extension below the sideburn ; a limited skin undermining is performedTwo strong , permanent purse - string sutures are woven into the superficial musculoaponeurotic system tissues in a vertical U and an oblique O shape , initiating from a strong anchorage in the deep temporal fascia at the level of the helical crusTying these sutures produces a very powerful vertical correction of descended facial features that acts mainly on the jowls and the upper neckThe procedure can be extended by contenting the dissection over the malar fat pad , placing a third vertical purse - string suture with strong action on the nasolabial groove , and vertically repositioning the midfacial volumes
