N2  - The treatment of proximal humerus fractures has been a subject of intense discussion with supporters of both operative and nonoperative methods. Several treatment modalities have been proposed depending upon the fracture pattern, patients’ age, level of activity and, bone stock, and associated medical co-morbidities: conservative treatment ORIF, percutaneous fixation and joint replacement. Our study conducted upon in elderly group with three parts and four parts , proximal humerus fractures according to Neer's classification patients was randomized to either who were treated with pinning and who were treated by ORIF with PHILOS plate; يعد علاج كسور عظم العضد القريبة موضوعًا للنقاش والجدل المكثف في القرن الماضي بين مؤيدي الطرق الجراحية وغير الجراحية. تم اقتراح العديد من طرق العلاج اعتمادًا على نمط الكسر، وعمر المريض، ومستوى النشاط، ومخزون العظام. أجريت دراستنا على 40 مريضًا من كبار السن . تم تقسيم المرضى بشكل عشوائي إلى مجموعتين. في مرضى المجموعة الأولى تم عمل تثبيت خارجي بواسطة السلوك المعدنية ، أما في مرضى المجموعة الثانية تم عمل تثبيت داخلي باستخدام شريحة فيلوس
