<br/><br/> Background: In case of tibial plateau fractures type II and III we found that functional and radiological results of using rafting screws were slightly lesser than when graft was used, but it avoided possible donor site complications of the graft, decreased blood loss and minimized operative time. <br/>Objective: To evaluate the clinical, radiographic and functional results in a cohort of patients who sustained open reduction and internal fixation by rafting screws 3.5- 6.5mm cortical screws. <br/>Patients and Methods: A total number of 54 cases were included in this study and divided into 2 equal groups: Group (A) and group (B). The demographic data showed sex distribution as males & females with closed lateral tibial plateau fractures (schatzker type II & type III). <br/>Results: As regard to radiological outcome, there was no significant difference between both groups during the period between December 2021 and November 2023 at Kasr Alainy hospital and Al-Helmya Military Hospital<br/>Conclusion: Treating depressed tibial plateau fractures type II and type III with mild or moderate depression (<1.5cm) by open reduction and internal fixation using large set buttress plate and screws or screws alone without using bone autograft represented a viable option.   تم تضمين إجمالي عدد 54 حالة في هذه الدراسه، منها 27 حالة تمثل المجموعة (أ) و 27 حالة تمثل المجموعة (ب). أظهرت البيانات الديموغرافية وجود 22 مريض من الذكور و 8 إناث مصابين بكسور مغلقة في الهضبة الظنبوبية الجانبية). و كان متوسط الإقامة في المستشفى 6.7 أيام لكلا المجموعتين مع عدم وجود فرق كبير بين المجموعتين. جميع المرضى لديهم كسر مغلق بدون إصابات مصاحبة. و أجريت الجراحة بعد يومًا في المتوسط من القبول.فى الفترة مابين ديسمبر 2021 حتى نوفمبر 2023
