<br/><br/> Background: Worldwide attention for postmastectomy shoulder dysfunction. <br/>Potential evidence regarding Maitland mobilization for its therapeutic benefits <br/>shoulder restrictions. Virtual reality is an innovative approach that facilitates <br/>simulation of functional tasks. Purpose: To determine efficacy of virtual <br/>reality and maitland mobilization on shoulder dysfunction postmastectomy. <br/>Method: Sixty patients of both genders (57 females, and 3 men) suffering from <br/>postmastectomy shoulder dysfunction from Demitta Governmental Hospital, <br/>Demitta, participated in this study and they were 40-65 years old. They were <br/>randomly allocated into three equal groups in numbers; Group A received <br/>Maitland mobilization, in addition to conventional physical therapy protocol <br/>(shoulder range of motion, and capsular stretching exercises); Group B <br/>received Virtual reality, in addition to conventional physical therapy protocol, <br/>and Group C received Maitland mobilization and Virtual reality, in addition to <br/>conventional physical therapy protocol. The study was conducted along 4-<br/>weeks. Outcome measures: Shoulder mobility using digital goniometer, and <br/>shoulder dysfunction using shoulder pain and disability index. Results: All <br/>groups had revealed a significant increase in shoulder mobility (18.03, 13.08, <br/>and 29.89% in flexion, 18.27, 8.27, and 24.81% in extension, 27.79, 17.57, and <br/>40.5% in internal rotation, and 28.93, 18, and 45.14% in external rotation for <br/>groups A, B, and C, respectively), reported values, and a significant <br/>improvement in shoulder dysfunction ( 31.7, 21.95, and 38.75% for groups A, <br/>B, and C, respectively), reported values post treatment. While, there was <br/>significant improvements with favor for group C, over group A, over group B <br/>posttreatment in term of shoulder mobility, and dysfunction. Conclusion: <br/>Adding maitland mobilization, and virtual reality, to conventional physical <br/>therapy program have beneficial effect on improving shoulder mobility, pain <br/>intensity, and functional capability.   الخلفية العلمية: تزايد الإهتمام العالمي الإعتلال الوظيفي للكتف بعد استئصال الثدي كنقطة انطلاق لشفاء متكامل. فمع تنامي دلالات استخدام العلاج بتقنية ميتلاند كعلاجات بديلاً، وكذلك العلاج بالواقع الإفتراضي لتعاظم فوائدهما العلاجية لعلاج الاعتلال الوظيفي للكتف بعد استئصال الثدي.<br/>الغرض: تم إجراء الدراسة لمقارنة الفعالية العلاجية لتقنية الواقع الإفتراضي مقابل تقنية ميتلاند علي المرضي ممن يعانون من الإعتلال الوظيفي لمفصل الكتف بعد استئصال الثدي. إجراءات الدراسة: أجريت الدراسة علي ستين مريضًا من كلا الجنسين (٥٧ سيدة، و٣ رجال) يعانون من الاعتلال الوظيفي للكتف بعد استئصال الثدي تتراوح أعمارهم من ٤٠-٦٥ عاماً تم تقسيم المرضي إلى: ثلاثة مجموعات عشوائياً: المجموعة الأولي تلقت العلاج بتقنية ميتلاند وبروتوكول العلاج الطبيعي النمطي-المجموعة الثانية تلقت العلاج بتقنية الواقع الافتراضي بالإضافة لبروتوكول العلاج الطبيعي النمطي، بينما المجموعة الثالثة فتلقت العلاج بكلا من تقنيتي ميتلاند والواقع الافتراضي مع بروتوكول العلاج الطبيعي النمطي علي مدي أربعة أسابيع متتالية مع التقييم قبل وبعد الدراسة باستخدام مقياسي مقياس المدي الحركي الرقمي مؤشر ألم والإعاقة للكتف وتبعه تحليل إحصائي تفصيلي. النتيجة: لم يتبين وجود فروقاً ذات دلالة إحصائية بالمؤشرات الديموجرافية والإكلينيكية لمجموعات الدراسة(٠.٠٥<). لكن تبين وجود اختلافات ذات دلالة معنوية مرتفعة بمقياس المدي الحركي للكتفو مؤشر الألم والإعاقة للكتف(٠.٠٥> ) بعد الإنتهاء من العلاج. الخلاصة: العلاج بكلا من تقنية ميتلاند وتقنية الواقع الافتراضي ذا دلالة وفعالية علاجية مرتفعة لعلاج حالات الإعتلال الوظيفي للكتف بعد استئصال الثدي. كما تبين أن إضافة تقنيتي ميتلاند والواقع الإفتراضي لبرنامج العلاج الطبيعي النمطي يضمن علاجاً ذا فعالية علاجية مرتفعة لحالات الاعتلال الوظيفي للكتف بعد استئصال الثدي.
