<br/><br/> Both anterior-to-posterior screw fixation using indirect reduction and posterior <br/>buttress plating via direct reduction through the posterolateral approach are viable <br/>options for stabilizing the posterior column in Pilon fractures involving two, three, or <br/>four columns, and both can result in successful fracture union. However, based on our <br/>findings, posterior plating provided superior reduction quality and yielded better <br/> overall functional outcomes as reflected by higher AOFAS scores.  كلا الطريقتين – التثبيت بالبراغي من الأمام إلى الخلف باستخدام الرد غير المباشر، والتثبيت بالصفائح الداعمة الخلفية باستخدام الرد المباشر يمكن استخدامهما لتثبيت العمود الخلفي في كسور بيليون التي تشمل عمودين أو ثلاثة أو أربعة أعمدة، وكلاهما يمكن أن يؤدي إلى التئام الكسر..<br/>لكن من خلال دراستنا، توصلنا إلى أن التثبيت بالصفائح الخلفية يوفّر ردًّا أفضل للكسر، ويؤدي إلى نتائج وظيفية أفضل بشكل عام، كما يتضح من ارتفاع درجات مقياس الجمعية الأمريكية لجراحي العظام للقدم والكاحل
