Aim: To compare the effect of a final irrigation with antimicrobial - corticosteroid mixture with cryotherapy on post-instrumentation pain and microbial reduction in patients with single canal teeth with necrotic pulps and asymptomatic apical periodontitis. Methodology: Twenty-eight patients having single canal teeth diagnosed with necrotic pulp and asymptomatic apical periodontitis were recruited to participate in the study. Root canal treatment was performed in two visits. At the first visit, access cavity was prepared, preoperative microbial samples (S1) were collected, mechanical preparation was performed using Hyflex CM files with 2.5% NaOCl irrigation, then post-instrumentation microbial samples (S2) were collected. Patients were then randomly allocated according to the final irrigation either to the “Mix group” receiving a mixture of levofloxacin, dexamethasone and fluconazole or to the “Cryo group” receiving cryotherapy, then the third microbial samples (S3) were collected. Patients were requested to record their pain intensity on a 10-point numerical rating scale (NRS) at 6,12, 24, 48 and 72 hours postoperatively. In the recall visit after 1 week, after confirming that patients were asymptomatic, canal irrigation was done with17% EDTA, saline and 2.5% NaOCl, then master cone fit and obturation were performed. Results: The two groups showed no significant increase in postoperative pain intensity at all time intervals compared to preoperative pain intensity. The two groups showed mild pain intensity at 6 and 12 hours postoperatively with no significant difference between the two groups. Moreover, no pain was reported in both groups at 24, 48 and 72 hours postoperatively. At the post-final irrigation microbial samples, both groups showed significant bacterial and candidal load reduction. vii Conclusion: There were no differences between the two groups regarding pain intensity, bacterial load and candidal load at all time intervals. In necrotic teeth with asymptomatic apical periodontitis, both the antimicrobial-corticosteroid mixture and the cryotherapy groups are suitable for use as a final irrigation after mechanical preparation in presence of 2.5% sodium hypochlorite to reduce postoperative pain and intracanal microbial loads. Clinical relevance: Teeth with necrotic pulps and asymptomatic apical periodontitis could be successfully treated with the use of the lower concentration (2.5%) NaOCl combined with either antimicrobial-corticosteroid mixture or cryotherapy with minimal postoperative pain intensity.
