Purpose: Needs to employ quality assurance (QA) programs that ensure that the system performance meets high expectations consistent with patient care requirements. And to improve target localization accuracy, so that PTV treatment margin can be adjusted or reduced. Materials and Methods: Recently installed an Elekta Synergy linear accelerator with energy 6MV (Elekta, Synergy S, XVI, Crawley, UK) system. Catphan 503 phantom to evaluate CBCT image quality. Ball-Bearing Phantom to evaluate the geometric accuracy of kV and MV isocenters. Quasar Penta-Guide Phantom for daily quality assurance (QA). Results: In the present study over a period of 12 months showed that, MV and kV isocenters with mean value of 0.10±0.29mm, -0.07±0.37mm, 0.04±0.28mm and the maximal deviation were 0.45mm, -0.55mm and -0.47mm in x (lateral), y (longitudinal) and z (height) respectively
